Plea of Not Guilty and Request For Hearing
This form allows a driver to enter a plea of not guilty and appear before the court with any evidence to support claim of innocent.

This form allows a driver to enter a plea of guilty/no contest with an explanation to a traffic citation without appearing in court. The judge will make a ruling without the driver being present.
