If you claim an optional exemption or mandatory disqualification listed below, you may be excused.<\/p>\n<h2><b><strong>Statutory reasons to be excused or disqualified<\/strong><\/b><\/h2>\n<ul>\n<li>70 years of age or older and wish to be permanently excused<\/li>\n<li>70 years of age or older and wish to be excused for this term only<\/li>\n<li>Not a legal resident of Collier County \u2013 driver\u2019s license with new address required<\/li>\n<li>Not a U.S. citizen \u2013 copy of alien card or visa<\/li>\n<li>Convicted felon, civil rights not restored<\/li>\n<li>Under prosecution for a crime<\/li>\n<li>Expectant mother<\/li>\n<li>Woman who has given birth within the 6 months before the term starting date on the summons<\/li>\n<li>Parent not employed full time who has custody of a child under 6 years of age<\/li>\n<li>Full-time care giver caring for someone unable to care for him\/her self<\/li>\n<li>Full time student between 18-21 years of age<\/li>\n<li>Full-time federal, state or local law enforcement or investigative personnel \u2013 Need badge number and agency<\/li>\n<li>Any person who, because of physical or mental incapacity is permanently incapable of caring for himself or herself may be permanently excused from jury service upon request if the request is accompanied by a written statement to that effect from a licensed physician. Such requests are subject to review by the court. Physician statements must be emailed to <a href=\"mailto:juryhelp@collierclerk.com\">juryhelp@collierclerk.com<\/a> prior to consideration.<\/li>\n<li><strong>Counsel &amp; practicing physicians seeking to be excused:<\/strong> Please be aware that pursuant to F.S. 403.13(5) your request will be forwarded to the Court for consideration.<\/li>\n<\/ul>\n<p><b><strong>A person who is summoned and has reported as a prospective juror is exempt from jury service for a minimum period of one year.<\/strong><\/b><\/p>\n<p>To submit requests for disqualification or excusal, visit the <a href=\"https:\/\/jury.collierclerk.com\">Jury Response Website<\/a> or call the Automated Jury Phone at <a href=\"tel:(855)%20675 -9300\">(855) 675-9300<\/a>.
