I think l.remember why they tagged my license to be "restricted to renovation of skate ramps." You'll see on the form that my pervious company verified "myself." I have actually never worked for any construction company but self-taught myself everything I needed to know. Also, I have always been my own boss when it comes to any project l've undertaken. Of course, I have always consulted my "elders" in the industry a few times if I was in a "pickle." I have been building for about 4O years now and feel fully confident when taking on any project. Here are 3 things I have never done in all my years working: I have never charged a customer for unfinished work. lf lfail to complete a task, the bill is "on me" My completion rate is probably about 98% I have never used deposits from one job to pay for another project and have used funds appropriately for each job. I feel that if I ever started to do that, I could risk falling behind, which could put my company in jeopardy. Finally, I have always warrantied any work that I have done and have no plansto move or "skip town" I estimate l've only had to go back or improy'e on absqt 5%.of qeiohs-Andwhon someone calls, l've always been there within a day or two. The scope of work includes the installation, repair, or replacement of existing equipment, any cleaning or equipment sanitizing which requires at least a partial disassembling, excluding filter changes, and the installation of new pool/spa equipment, interior finishes, the installation of package pool heaters, the installation of all perimeter piping and filter piping, and the construction of equipment rooms or housing for pool/spa equipment, and also includes the scope of work of a swimming pool/spa servicing contractor. The scope of such work does not include direct connections to a sanitary sewer system or to potable water lines. The installation, construction, modification, or replacement of equipment permanently attached to and associated with the pool or spa for the purpose of water treatment or cleaning of the pool or spa requires licensure; however, the usage of such equipment for the purposes of water treatment or cleaning shall not require licensure unless the usage involves construction, modification, or replacement of such equipment. Water treatment that does not require such equipment does not require a license. In addition, a license shall not be required for the cleaning of the pool or spa in any way that does not affect the structural integrity of the pool or spa or its associated equipment. Juan Carlos Lopez Lopez, dba Huistas lnfinite Masonry LLC (hereinafter "Applicant") is before the Board for review of his credit score and a determination of whether the Applicant's credit score should prevent the licensee from having his probation continued or revoked. The Board, having heard testimony under oath, received other evidence, and heard arguments relative to all appropriate matters thereupon, issues its Findings of Fact, Conclusions of Law and Order of the Board as follows: FINDINGS OF FACT 1. The Applicant applied to the Collier County Contractor Licensing Supervisor, or his designee, for issuance of a license as a specialty license of masonary Contractor. 2. The Applicant's application was reviewed by the Board at its June 19, 2OZ4 meeting. The Applicant was granted a license under certain conditions. Any time during the probation period the Applicant may submit a credit report showing a score of at least 660 and the probation will be lifted. The Applicant shall provide a credit report to the Contractor Licensing Supervisor, or his designee, at twelve (12) months. lf the credit report is 660 or greater the probationary period shall be automatically lifted. lf the score has not improved to at least 660 the Applicant shall appear back before this Board to explaln the reason(s) the score is below the 660 minimum requirement." 3. The Applicant was present at the public hearing and was not represented by counsel. 4. The Applicant's credit score has improved but still does not meet that 660 credit score level. 5. The Applicant has demonstrated to the Board's satisfaction that the Applicant has improved his creditworthiness to allow an extension of the probationary period. CONCLUSIONS OF LAW 1. All notices required by the Code of Laws and Ordinances of Collier County, as amended, have been properly issued and the Board has jurisdiction over the matter. 2. Pursuant to Section 22-184(b) of the Code of Laws and Ordinances of Collier County, as amended, the subject application met the requirement(s) requiring Board review and approval of the application. 3. Sam Rothman was unaware of MDT Electric's license status at the time as Theodore Wynn provided fraudulent licensing documents to him and Holiday Manor management. Theodore Wynn was subsequently arrested by the Collier County Sheriff's Office and remains in custody. I spoke with Sam and explained that the permit was never issued and therefore never inspected. Sam stated Theodore Wynn provided him with an "inspection worksheet' showing that the system had passed inspections. He later provided this documentation. Shelby told me the LED security lights were for the main office where she is located. She further stated the 100 AMP riser was located on Duncan LN between spaces 103 and 105 but she did not know which space had the riser. I noted the exterior LED security lights were for the front, east side and back of the building. The invoice was dated 01-07-2025 for $990.00. It was unclear which lights had been installed by MDT Electric as the invoice has only a vague description. I then went to Duncan LN and spoke with Sam Rothman at 103 Duncan LN (828 385- 3420). Sam told me MDT Electric installed a new 100 AMP riser for his space in or around July 2025. He stated MDT Electric provided him with a permit number and located an email from twynn@mdtelectric.com with permit application PREL20250626066 (attached) dated 06-16-2025. Research shows the permit is "INVALID LICENSE" status and shows a pending inspection. The invoice for this project was dated 01-14-2025 for $1,025.00.
