Agenda 01/27/2026 Item #16D 3 (Second Amendment Subrecipient Agreement between Collier County and Collier Health Services, Inc., d/b/a/ HealthCare Network)1/27/2026 Item # 16.D.3 ID# 2026-3 Executive Summary Recommendation to approve and authorize the Chair to sign the Second Amendment Subrecipient Agreement between Collier County and Collier Health Services, Inc., d/b/a/ HealthCare Network, to amend Community Development Block Grant Agreement #CD-CV21-09 to revise the project scope, components, and payment deliverables (Housing Grants Fund 1835, CDBG-CV Project 33674). OBJECTIVE: To provide a collaborative community healthcare plan in support of Collier County’s Strategic Plan to provide access to health, wellness, and human services. CONSIDERATIONS: The U.S. Department of Housing and Urban Development (“HUD”) provides Community Development Block Grant (“CDBG”), HOME Investment Partnerships Program (“HOME”), and Emergency Solutions Grant (“ESG”) entitlement funds to the County annually to address program-specific needs in the community. The Board of County Commissioners (“Board”) previously approved the 2019-2020 One Year Action Plan on June 25, 2019 (Agenda Item #16. D.2). The 5-Year Consolidated Plan and Citizen Participation Plan were approved by the Board on June 28, 2016 (Agenda Item #11. E). On September 22, 2020, the County received notification of CDBG-CV funding in the amount of $1,561,633. On April 27, 2020, the County Manager signed the substantial amendment to Collier County’s U.S. Department of Housing and Urban Development Annual Action plan for FY2019-2020 to address additional funding and programs related to the pandemic. On May 12, 2020 (Agenda Item #16. F.1(f)), the Board ratified the County Manager’s action to amend the 2019-2020 One Year Action Plan to recognize the first round of CDBG-CV funding. On February 5, 2021, the County received notification of the second CDBG-CV funding allocation in the amount of $2,671,095. On November 10, 2020 (Agenda item #16. D.8), the Board approved the second substantial amendment to the 2019- 2020 Action Plan recognizing the second allocation of CDBG-CV funding, bringing the total award for Collier County to $4,232,728 respectively. On May 28, 2024 (Agenda item #16. D.4), Community and Human Services (“CHS”) amended the 2019-2020 Annual Action Plan for CDBG, CDBG-CV, ESG, and HOME funding to recognize the reallocation of funding within identified programs to be undertaken and respond to community needs. On April 8, 2025 (Agenda Item #16. D.8), the Board approved Subrecipient Agreement #CD-CV21-09 with Collier Health Services, Inc. d/b/a Healthcare Network (“Collier Health Services”), in the amount of $225,000.00, to support the development of a community health plan addressing healthcare access, preventative care and responsiveness to help mitigate the risk of future public health emergencies within the community. The plan includes partnerships with the health department and local medical providers/facilities. On September 23, 2025 (Agenda Item #16. D.1), the Board approved the First Amendment with Collier Health Services to revise the plan in order to reduce duplication of efforts with the Florida Department of Health in Collier County. On November 18, 2025, Collier Health Services requested a second realignment of the project plan, requiring an amendment to the Subrecipient Agreement. The amendment supports the renaming of the plan to Response and Engagement for At-risk Community Health (“REACH”). Importantly, the work completed to date remains relevant, valuable, and fully applicable to the revised project direction, providing a foundation for the updated scope, components, and deliverables. The proposed Second Amendment will revise the agreement by: a) Update the plan name to Response & Engagement for At Risk Community Health (REACH) into the scope of work and throughout the agreement to add clarity. b) Part 1 Scope of Work- Update the project components to support the plan’s transition from an inter-agency Page 4267 of 5261 1/27/2026 Item # 16.D.3 ID# 2026-3 emergency response implementation to an intra-agency approach. Section 1.1 Grant Special Conditions a. Project Details E. Project Description/Project Budget: Update the project components description to support the plan’s shift. H. Payment Deliverable: Update the payment deliverables description to support the plan’s shift. This item is consistent with the Collier County strategic plan objective by providing access to health, wellness, and human services. FISCAL IMPACT: There is no new fiscal impact. Funding for this agreement will be paid by funds from Department of Housing and Urban Development (“HUD”) residing within Housing Grant Fund (1835), Project 33674 (CBDG-CV). GROWTH MANAGEMENT IMPACT: This item has no impact on the Housing Element of the Growth Management Plan of Collier County.
