A RESOLUTION OF THE BOARD OF COUNTY COMMISSIONERS OF
COLLIER COUNTY, FLORIDA, REAPPOINTING ROSEMARY LEBAILLY
AND APPOINTING TABATHA BUTCHER TO THE COLLIER COUNTY
EMERGENCY MEDICAL SERVICES POLICY ADVISORY BOARD.
WHEREAS, Collier County Ordinance No. 2009 -01, as amended, established the Collier County
Emergency Medical Services Policy Advisory Board to assist the Board of County Commissioners
with all issues affecting prehospital emergency medical services within all of Collier County; and
WHEREAS, Ordinance No. 2009 -01, as amended, provides that the Board shall be composed of
five members representing the professions related to emergency medical services as specified in the
Ordinance; and
WHEREAS, there are currently vacancies on this Advisory Board; and
WHEREAS, the Board of County Commissioners previously provided public notice soliciting
applications from interested parties; and
WHEREAS, the Collier County Emergency Medical Services Advisory Policy Advisory Board
has provided the Board of County Commissioners with its recommendations for appointment.
NOW, THEREFORE, BE IT RESOLVED BY THE BOARD OF COUNTY
COMMISSIONERS OF COLLIER COUNTY, FLORIDA, that ROSEMARY LEBAILLY, in the
category of emergency room nurse, is hereby reappointed to the County Emergency Medical Services
Policy Advisory Board for a four -year term, said term to expire on September 25, 2015, and
TABATHA BUTCHER, in that category of EMS representative, is hereby appointed to the County
Emergency Medical Services Policy Advisory Board to fulfill the remainder of an unexpired term, said
term to expire on September 25,2013.
THIS RESOLUTION ADOPTED after motion, second and majority vote on this the 13th day of
December, 2011.
