RESOLUTION NO. 93-~
A RESOLUTION APPOINTING MEMBERS TO THE COLLIER
COUNTY PUBLIC HEALTH UNIT ADVISORY BOARD
WHEREAS, the Board of County commissioners adopted Collier
County ordinance No. 93-2 creating the Collier county PUblic
Health Unit Advisory Board; and
WHEREAS, Collier County Ordinance No. 93-2 provides that the
Collier County Public Health Unit Advisory Board shall consist of
seven (7) members, one me~ber from each of the five County
Commission Districts and two members at-large; and
WHEREAS, the Board of County Commissioners previously
provided public notice soliciting applications from interested
parties.
NOW, THEREFORE, BE IT RESOLVED BY THE BOARD OF COUNTY
COMMISSIONERS OF COLLIER COUNTY, FLORIDA, that the following
members are hereby appointed to the Collier County Public Health
Unit Advisory Board:
Pamela Co~, representing County commission District No. 1
Dr. Denise Heinemann, at-large member
Dick Mount, representing County commission District No. 3
Charles Rhoades, representing County Commission District
No.4
Dr. william Lascheid, at-large member
Ann Campbell, representing County Commission District No. 2
James Penczykowski, representing County Commission District
No. 5
The Collier County Public Health Unit Advisory Board members
shall decide upon the initial terms of office, as set forth in
Ordinance No. 93-2, for the seven (7) members appointed by the
Board at County commissioners and shall recommend to the Board of
County commissioners the initial terms of office for each of the
seven (7) members. The Board of County Commissioners by separate
Resolution shall approve the initial terms of office as it deems
appropriate.
