A RESOLUTION REAPPOINTING ANN R. CAMPBELL
AND CHARLES R. RHOADS TO THE COLLIER
COUNTY PUBLIC HEALTH UNIT ADVISORY BOARD
WHEREAS, the Board of County Commissioners adopted Collier
County Ordinance No. 93-2 creating the Collier county Public
Health Unit Advisory Board; and
WHEREAS, Collier county Ordinance No. 93-2 provides that the
Collier County Public Health Unit Advisory Board shall consist ot
seven (7) members, one member trom each ot the tive county
Commission Districts and two members at-large; and
WHEREAS, the terms of two (2) of the members representing,
commission Districts 2 and 4 will expire on March 15, 1994; and
WHEREAS, the Board of County commissionors previously
provided public notice soliciting applications from inter.sted
parties.
NOW, THEREFORE, BE IT RESOLVED BY THE BOARD OF COUNTY
COMMISSIONERS OF COLLIER COUNTY, FLORIDA, that:

1. Ann R. Campbell, representing County Commission District
NO.2, is hereby appointed to the Collier County Public Health
Unit Advisory Board for a term of three (3) years, said terll
expiring on March 16, 1998.
2. Charles R. Rhoads, representing county commission
District NO.4, is hereby appointed to the Collier County Public
Health Unit Advisory Board tor a term of three (3) years, said
term expiring on March 16, 1998.
This Resolution adopted after motion, second and majority
