Resolution 1999-408 !,.""',... ' 16C 7 . RESOLUTION NO. 99- 4 DB RESOLUTION ESTADLlSIIING POLICIES AND PROCEDURES FOR INDIGENT BURIALS AND CREMATION WHEREAS, Chapter 245. Flornla Slall/le.f. establishes criteria for the disposition of persons who expire within the confines of every counly, city, village, town or municipality in Florida; and WHEREAS, The Department of Collier County Social Services is responsible for establishing eligibility and for the authori7.1lion of payment for burial or cremation of indigenl persons who expire within the confines of Collier Counly if the deceased is unclaimed or survivors have insufficient resources for burial or cremalion; and WHEREAS, The Board of County Commissioners has determined thai a policy should be created establishing the allowable fees for such services; and NOW, THEREFORE, BE IT RESOLVED BY THE BOARD OF COUNTI' COMMISSIONERS OF COLLIER COUNTY. FLORIDA,that: I. EJ.lGlnILlTY: Any person declared indigent, resident or non-residenl, who expires within the con fanes of Collier CounlY, may be eligible for county buriaVcremation. Reasonable efforts will be made to determine the financial situation oflhe deceased and family members. Collier County shall not authorize payment to transport a deceased to another county or state for disposition unless specifically authorized as a public purpose by the Board of County Commissioners. The body of a deceased may be lransported out of the counly If the body has been accepted by the Slate of Florida Anatomical Board for use in research and leaching. 2. PROCE()URF:S: a. Collier County will make a reasonable effort to gal her pertinent informalion, including name, date of birth, Social Security number, dale of death, cause of death, next of kin, financial information, veleran stalus. willingness ofrelalives to donate the body for research for education and preference for burial or cremation. In order to proceed with cremation, a family member must be willing to sign an authorization for cremation and disposition for the funeral home. Oraveside services will only be provided upon request. c. 1 f identification of lhe body has not been established, burial shall be delayed a minimum often (10) days from the omcial date of death. lfidentification of the body has been established and the next of kin has not been found, lhe body will be held for ten (10) days from the omcial dale ofdcath ,nd until reasonable elTorts 10 locate next of kin have been exhausted, d. The following procedures will be followed by all participating funeral homes for burial/cremation of in dig en I deceased, (i) [Jurial ofindigent ,dults shall require a minimum of a manufactured pressed board cloth covered casket and a concrete liner. (ii) At the discretion of the Funeral Director, burial of in dig en I decomposed bodies may require a metal sealed unit similar 10 the brand name Ziegler, wilhout a casket; a concrete liner shall be required for inlerment (iii) Burial of in dig en I still born and other infants shall require a casket vault, dome sealed, fiber plastic, weather proofed casket A concrcte liner shall not be required for inlerment. (iv) The funeral director shall provide embalming or cremation, clothing. transportation, grave markers, and wilt secure all necessary legal documents. (v) The Veteran'. Service omce will be responsible for order- ing a grave markcr for indigent vel crans. Markers will be shipped 10 the Park. and Recreation Department whose stalTwill be responsible for installation at the proper grave site within 15 days of receipt e. Burial of Collier County indigent cases shall be performed with strict adherence 10 lhe above statcd proccdures. If deviation from established procedure appears neccssary. lhe appointed provider of service shall notify lhe Social Services Department and the department shall determine ifany deviation from the established procedure is appropriate in such case, r. The funeral home providing .ervice for an indigent deceased .hall advise the Social Services Department of lhe desired date and lime for burial. The Social Services Department shalllhereupon reque.t from the Real Property Departmenl provision of burial space and opening and closing of the grave. Afier disposition ofthe indigcnl deceased has been accomplished, an ilemized Slalement requcsting payment shall be presented to the Social Services Department by lhe funcral home that pro\'ided the service. Collier Counly shall provide payment 10 the funeral home according to the fee schedule in effect at the timc serviccs were rendered. The current fee schedule is attached as Exhibil A and included herein. Paymenl will be approved by the Social Services Director ifno irregularity appears, lhe bill will he senlto the Clerk ofC'ollrts Finance Dcpartment for payment.
