RESOLUTION NO, 96-208
A RESOLUTION APPOINTING AND REAPPOINTING
MEMBERS TO TIlE COLLIER COUNTY PUBLIC
HEALTH UNIT ADVISORY BOARD.
WHEREAS, the Board of County Commissioners adopted Collier County Ordinance No.
93-2 creating the Collier County Public Health Unit Advisory Board; and
WHEREAS, Collier County Ordinance No. 93-2 provides that the Collier County Public
Health Unit Advisory Board shall consist of seven (7) members, one member ITom each of the five
County Commission Districts and two members At-Large; and
WHEREAS, the terms of three (3) members, representing Commission Districts 3 and S,
and one At-Large category expired creating vacancies on this Board; and
WHEREAS, the Board of County Commissioners previously provided public notice
soliciting applicationa ITom intereated parties;
NOW, THEREFORE, BE IT RESOLVED BY THE BOARD OF COUNTY
COMMISSIONERS OF COLLIER COUNTY, FLORIDA, that:
1. Denise Skelly Heinemann, representing the At-Large category, is hereby reappointed to
the Collier County Public Health Unit Advisory Board for a three (3) year term, said term
expiring March 16, 1999.
2. James L. Penczykowski. representing Commission District S, is hereby reappointed to the
Collier County Public Health Unit Advisory Board for a three (3) year term, said term expiring
March 16, 1999.
3. Patricia A. Meyers, representing Commission District 3, is hereby appointed to tho
Collier County Public Health Unit Advisory Board for a three (3) year term, said term expiring
March 16, 1999.
This Resolution adopted after motion, second and majority vote.
