RE:SOLUTION NO. 69 - 24
RE:SOLUTION FOR APPOINTME:NT TO
THE: COLLIE:R COUNTY HE:ALTH FACILITIE:S AUTHORITY
JANUARY 31, 1989
WHEREAS, the Board of County Commissioners created the Health
on November 20, 1979; and
Facilities Authority of Collier County by County Ordinance No. 79-95
WHEREAS, the Board appointed certain individuals as the
members of the Authority; and
WHE:RE:AS, the term of Harry E. O. Heine~an expired on December
17, 1966, and the term of Jane Polkowski, M.D. expired on December
16, 1966; and
WHE:RE:AS, the Board desires to reappoi~t Dr. Polkowski to an
additional term of four years to rUn from the date of the expiration
of the previous term; and
WHEREAS, Mr. Heineman has indicated he does not wish to be
reappointed. and Mr. John W. Bush desires to be appointed to the
Au thor ity.
NOW, THEREFORE, BE: IT RE:SOLVE:D BY THE: BOARD OF COUNTY
COMMISSIONE:RS OF COLLIE:R COUNTY, FLORIDA that:
1. The following individuals are hereby appointed or
reappointed respectively to the Health Facilities Authority to serve
a term of four years in accordance with statutes, which term will
expire as show below:

2. This Resolution shall constitute a certificate of
appointment and shall be filed with the Clerk of the Circuit Court
as provided by law.
This Resolution adopted this 31st day of January, 1969.
