Before me, the undersigned authority, personally appeared Gary A. Myers, who being by me
first duly sworn, on oath deposes and says that:
1. He is the President on behalf of Hyde Park At Pelican Bay Condominium
Association, Inc., a Florida not-for-profit corporation.
2. Said corporation is currently in existence under valid Articles of Incorporation and
Bylaws and has not been terminated or dissolved.
3. That he is authorized by the Articles of Incorporation and/or Bylaws to execute
deeds, easements and mortgages on behalf of the corporation, and all
necessary consents have been obtained.
4. The Corporation is not currently in any bankruptcy proceedings and is not a
debtor in bankruptcy.
5. Affiant further states that he is familiar with the nature of an oath and with the
penalties as provided by the laws of the State of Florida for falsely swearing to
statements made in an instrument of this nature. Affiant further certifies that he
has fully read this affidavit and understands its contents.
6. This Affidavit is given to induce Commonwealth Land Title Insurance Company to
issue its title policy insuring the contemplated transaction
