This Amendment is entered into this 1 `1 day of S t• 2021, by and between Collier County, a
political subdivision of the State of Florida,("COUNTY"o "Grantee")having its principal address at 3339
E Tamiami Trail, Naples FL 34112, and Immokalee Fire Control District, ("Subrecipient"), having its
principal office at 502 New Market Road East Immokalee, FL 34142.
RECITALS
WHEREAS, on June 23, 2020, the COUNTY entered into an Agreement for awarding
Community Development Block Grant Program funds to be used for the Immokalee Fire Station
Construction (hereinafter referred to as the"Agreement"); and
WHEREAS, the parties desire to clarify the scope of work, modify the project budget, correct the
period of performance end date in the reference table on page one, and correct the CHS contact information.
NOW, THEREFORE, in consideration of foregoing Recitals, and other good and valuable
consideration, the receipt and sufficiency of which is hereby mutually acknowledged, the Parties agree to
amend the Agreement as follows:

SCOPE OF WORK
Project Name: Immokalee Fire Station Construction
Description of project and outcome:
CHS, as administrator of the CDBG program, will make available CDBG funds up to the gross
amount of$4490,000$1,016,646.57 to the Immokalee Fire Control District to fund: The partial
operational, minus the living quarters area. The cost may include but not limited to the
construction and associated cost for permitting, mobilization, engineering, and inspection services
of the proposed new fire station in Immokalee (Collier County is not paying 100% of the project
cost). SUBRECIPIENT anticipates receipt of a State awarded approximately in the amount of
$900,000 toward the project. Should this State funding not be received,this Agreement may be
terminated in accordance with Section 3.08 of this Agreement.
Construction must be completed within five (5)years after the CDBG 2017-18 funded property
acquisition of 510 New Market Rd. E. in Immokalee, Florida(March 27, 2019) or shall replay all
funds from the land acquisition, design and construction award provided.
The property will be deed restricted for five(5)years commencing on the date of initially meeting
one of the National Objectives, in accordance with 24 CFR 570.505, if applicable.
