should be provided to the County Attorney Office at the time the item is input into SIRE.
Some documents are time sensitive and require forwarding to Tallahassee within a certain
time frame or the BCC's actions are nullified. Be aware of your deadlines!

FIRST AMENDMENT TO AGREEMENT#18-7343-WV
FOR COURT REPORTING
THIS FIRST AMENDMENT is made and entered into on this 11‘ CR. , oCY
by and between U.S. LEGAL SUPPORT, INC. (the "Vendor") and Collier County, a poical
subdivision of the State of Florida, (the "County")(collectively,the"Parties"):
WHEREAS,on May 8,2018,(Agenda Item 16.K.1),the County entered into Agreement#
18-7343-WV (the "Agreement') with Vendor to provide court reporting services for Collier County
and the Office of the Collier County Attorney; and
WHEREAS,the Agreement has an initial three-year term with two one-year renewal terms;
and
WHEREAS, the Vendor has not sought to increase its schedule of rates during the course
of the last three years but wishes to revise its rates to be effective during the two remaining one-year
renewal terms under the Agreement; and
WHEREAS, the Parties desire to amend the Agreement to revise and replace Exhibit B,
Fee Schedule,to recognize price adjustments to become effective during the first renewal period.
NOW, THEREFORE, in consideration of the mutual promises and covenants herein
contained, it is agreed by the Parties as follows:
1. Exhibit B,Fee Schedule,to the Agreement is hereby removed in its entirety and replaced
with Exhibit B, Fee Schedule, attached hereto and made a part hereof.
2. Except as amended herein, all other terms and conditions of the Agreement, which is
incorporated herein by reference, shall remain unchanged.
IN WITNESS WHEREOF,the Parties have executed this First Amendment by an authorized
person or agent on the date and year first written above.
