A RESOLUTION OF APPOINTMENT TO THE COLLIER COUNTY
HEALTH FACILITIES AUTHORITY
WHEREAS. the Board of County Commissioners of Collier County
creat~d the Collier County Health Facilities Authority (the
"Authority") by Ordinance No. 79-95 on November 20, 1979; and.
WHEREAS. the Board has appointed certain individuals as
m~mb~rs ~f th~ Authority; ~nrl.

WHEREAS, the term of offic~ ~! thr~~ ~~mb~rs has ~xpired:
WHEREAS, the Board desires ~o appoint the individuals named
