EXECUTIVE SUMMARY
Recommendation to approve the FY20-21 contract with the State of Florida Department of Health
for the operation of the Collier County Health Department in the amount of $1,491,500.
OBJECTIVE: To approve the FY20-21 Collier County Health Department's annual core contract.
CONSIDERATIONS: Each year, in accordance with Chapter 154, Florida Statutes, the County enters
into a core contract with the State of Florida Department of Health to provide funds and facilities for the
provision of public health services. The attached represents the FY20-21 contract effective October 1,
2020 through September 30, 2021.
The contract identifies the services that will be provided as well as the estimated cost-share between the
State and the County. The total amount of funding to be provided directly to the Department of Health in
Collier County under the contract is $1,491,500. The County also provides $377,900 for Health
Department facility and motor pool operating costs. In addition, the contract contains $2,577,376 of
COVID-19 funding received from the State.
FISCAL IMPACT: The County General Fund (001) budget provides for the base County-Health
Department contract of $1,491,500 as well as $377,900 to provide for Health Department facility and
motor pool operating costs.
LEGAL CONSIDERATIONS: The contract is a standard form prepared by the State Department of
Health. This Item is approved for form and legality, and requires a majority vote for Board approval. -

GROWTH MANAGEMENT IMPACT: There is no Growth Management Plan impact associated with
this Executive Summary.
RECOMMENDATION: To approve and authorize the Chairman to execute the annual core contract
with the State of Florida Department of Health for the operation of the Collier County Health Department.
