Agenda 07/14/2020 Item #16D 6 (After-the-fact Submittal (5) Applications to DEO)16.D.6 07/14/2020 EXECUTIVE SUMMARY Recommendation to approve an "After -the -Fact" submittal of five (5) applications to the State Department of Economic Opportunity for competitive Community Development Block Grant - Mitigation funding in the amount of $6,620,174. OBJECTIVE: To obtain State Department of Economic Opportunity (DEO) Community Development Block Grant Mitigation (CDBG-MIT) Critical Facilities Hardening Program funds for the mitigation and hardening of critical facilities throughout Collier County. CONSIDERATIONS: In response to Presidentially declared disasters in 2016 and 2017, Congress appropriated funding for the CDBG-MIT program to mitigate future disasters. This CDBG-MIT program has three (3) phases - Critical Facility Hardening Program (CFHP), General Planning Support, and General Infrastructure Program, all of which focus on mitigation of future disasters including hurricanes, wildfires, and other natural disasters. In response to previous disasters, the U.S. Department of Housing and Urban Development (HUD) is providing federal funding to the State of Florida DEO to harden critical public facilities to mitigate and become more resilient to future disasters. On April 15, 2020, the DEO announced the first round of CDBG-MIT funding. The total state allocation available for CFHP is $75,000,000. The DEO requires the unit of local government to serve as the applicant and coordinate applications on behalf of the local municipalities, healthcare providers, and other public agencies. This first round of CDBG-MIT applications was due to DEO on June 30, 2020. Due to the short turnaround time and need to coordinate grant applications with other public agencies and County Divisions, this countywide application is utilizing CMA #5330 which authorizes the County Manager to approve the submittal of grant applications followed by after -the -fact approval by the Board of County Commissioners (Board) at the next available Board meeting. The County Manager approved the applications on June 29, 2020, and they are attached to this Item. CDBG-MIT projects must meet the National Objective of benefiting low to moderate -income persons (LMI). The proposed projects were advertised in the Naples Daily News on June 14, 2020. There were no public comments received. A public meeting with invited local jurisdictions, public agencies, health care organizations, and the Seminole Tribe was held on June 1, 2020. The County is required to be the applicant, and should the funds be awarded, the County will sub -award to the HealthCare Network of Southwest Florida. CDBG-MIT funding does not require a match and is federally funded 100%. Project Name Description Amount of Funds Requested 1. Fether Medical Clinic Immokalee - Healthcare Network of SW Florida Public medical facility hardening to include removal and replacement of 65 windows and doors with code -compliant insulated/impact windows and doors. $ 216,552 5. Healthcare Network of SW Public facility hardening at three (3) $ 475,965 FL - Dental Care East clinic locations in Naples including medical and (Heritage Trail); Senior dental facilities. Replacement of 66 Friendship Health Center windows and doors with code -compliant (Stanford Court); and insulated/impact windows and doors. Children's Care East (Davis Blvd) TOTAL FUNDING $6,620,174 REQUEST FISCAL IMPACT: At this time there is no Fiscal impact to the County. The total project cost is $6,620,174. CDBG-MIT funding does not require a match and is federally funded 100%. If any or all of these projects are selected for funding by DEO, the Community & Human Services Division (CHS) will be eligible for a five percent (5%) administration fee for each funded project. Funds would be appropriated into CHS Fund (705). Local funds in the amount of $175,000 will be used for furniture, fixtures, and equipment as part of the Golden Gate Senior Center hardening project. These items are not eligible under the CDBG-MIT grant program. Funds are available in Countywide Capital Project Fund (301) Golden Gate Senior Center Rehab project supported by a transfer from the General Fund (001) earmarked within the FY21 proposed budget. Grant award agreements and Budget Amendments will be brought to the Board at a future date. LEGAL CONSIDERATIONS: The Board will have the opportunity to accept or reject the funds if the grant is approved. Accordingly, this Office has no issue with respect to the legality of this request, which is appropriate for Board action and requires a majority vote for Board action. -JAB GROWTH MANAGEMENT IMPACT: Award and implementation of these grants will further the Goals, Objectives, and Policies of the Capital Improvement, Recreation and Open Space, and Intergovernmental Coordination Elements of the Growth Management Plan. Powered by Qualtrics Z Cover C014"ty Community Development Block Grant - Mitigation (CDBG-MIT) — Immokalee Branch Library Project Description: The Immokalee Library is recognized by the community as not only a library, but a gathering place located in the center of the community. The 8,000 square foot public facility is surrounded by parks, public schools, County offices and is easily accessible through a major roadway. Throughout the year, the facility provides educational and enrichment opportunities to all ages from a diverse rural community with a significant low-income and underserved population. The building and its staff provide users with traditional library services including access to a print collection numbering 31,038. However, the importance of the facility goes beyond the number of books. To further meet the needs of the community, the county provides a wide range of educational and civic programming. Classes focus on literacy, technology, employment, citizenship, and the arts and are all critical in improving the quality of life for members of the community. The proposed project will not only protect the books, computers, and other assets inside the structure, but it will allow for their continued use soon after an event. With completion of the hardening and back-up power components of the project, the potential for heat and water damage will be greatly reduced. While the primary purpose of the Library is to serve the educational and civic needs of the public throughout the year, the facility plays a critical role in the recovery of the community following natural disasters. Using the building as a post -disaster recovery center have become a priority and part of the County's emergency management planning efforts. The building size and central location, close to the public schools and other County buildings that are part of the community's recovery efforts, is critical in meeting the needs of the community rapidly after an event. The hardening of the structure with a new roof, windows, and doors as well as installing a reliable source of back-up power means that the facility may better withstand a wind event, and the County can count on using the facility immediately after a disaster. With identified problems mitigated, the Library can provide rapid relief with functions such as providing water and food distribution, cooling areas, device charging areas, and access to information and recovery assistance. The proposed hardening project will mitigate water damage caused by intrusion through non -impact resistant windows and doors. A new roof will further mitigate water damage and protect the building against 180 mph wind speeds. The installation of a permanent stand-by generator and fuel source will protect the facility against power disruption and extended periods of electricity loss. The proposed project will be managed by the County's Facilities Management Division under the direction of a project manager in coordination with the Library Division and Community and Human Services Division. Execution and completion of the project will be through a formal public solicitation process, an evaluation of the respondent's qualifications and experience, and lowest responsible and reasonable bid. Estimated costs of the hardening of the Library's roof, windows, doors, and the installation of a permanent stand-by generator and fuel source is based on the Library's prior application for a Hazard Mitigation Grant Program (HMGP) application after Hurricane Irma. This application was recently denied due to not being cost effective for the purposes of the HMGP. Annual estimated costs for the maintenance of the wind retrofit doors and windows of the Library will be part of the annual preventive maintenance budget of the County's Facilities Management Division. Estimated annual costs for caulk, power washing, cleaning and basic repairs is $2,000. The Estimated annual cost of maintaining the permanent stand-by generator is $5,000 and would be incurred by the County's Facilities Management Division. Page 12 Community Development Block Grant - Mitigation (CDBG-MIT) — Golden Gate Senior Center Cover C014"ty Community Development Block Grant - Mitigation (CDBG-MIT) — Immokalee Branch Library Community Value Narrative: The Immokalee Branch Library includes 4,000 SF built in 1978 and a 4,000 SF expansion built in 2005. The 1978 structure was not retrofitted to the 2005 building code putting the doors and windows in that section of the building at risk for severe damage in the event of a major storm. New shingles were installed across the entire building during the expansion project in 2005, but at 15 years the shingles are now in need of replacement. This project would harden all windows and doors throughout the building bringing them up to current code for wind and impact resistance and will upgrade the roof to metal. Such efforts will reduce the risk and cost of repair or rebuilding of exterior structural damage and/or interior structural, asset, and resources damage arising from a major natural event. Should this project not be funded and completed, the Library is subject to increased vulnerability to natural and human -caused threats and hazards. The duration of these adverse consequences jeopardizes the resiliency and recovery efforts of the Immokalee community. Given the centralized location of the Library and the Library's status within the community as a safe and reliable resource, facility hardening and generator installation is paramount to the community's recovery efforts. The hardening and generator installation at the Immokalee Branch Library directly benefit the Safety and Security Lifeline. During recent recovery efforts the Library has been used for providing office space for local government offices including the Bureau of Emergency Services, state and federal agencies including FEMA. Along with office space, the Library is able to provide the agencies, organizations, and community with access to cooling areas; device charging areas; and computer, WiFi, and printing capabilities all within a centralized location of Immokalee. There is no known historical or cultural significance at the Immokalee Branch Library. 00*7 C014"ty Community Development Block Grant - Mitigation (CDBG-MIT) — Immokalee Branch Library Capacity Plan: The Immokalee Branch Library plays a critical role in the resiliency of the community. Numerous agencies and community partners look to the Library before, during, and after emergency events to provide valuable information, resources, and access to services for resiliency and recovery efforts. Specific stakeholders beyond the low to moderate income residents of the Immokalee community are: o Federal and State Agencies (including FEMA) o Collier County Government and Constitutional Offices (Including Bureau of Emergency Services) o Immokalee Chamber of Commerce o Local Business Leaders o Collier County Public Schools o Redlands Christian Migrant Association (RCMA) o Local Non -Profit Organizations (for example, Benison Center, Coalition of Immokalee Farmworkers, etc.) The project will consist of retrofitting the facility with 25 replacement windows, replacement of nine doors, including the front entrance, replacement of the existing roof and installation of a new permanent generator at the facility. The scope of work for the original HMGP project is to complete final design. Permitting, along with several vendors/contractors to complete the project elements. The design and engineering of the project will be limited in scope. The primary emphasis for the plans will be the windows and door retrofit, the installation of the generator, and fuel source. However, final plans will be required to obtain permits and to ensure that the installation of the project elements are done effectively and with no negative impact to the environment. The County will procure for an engineering firm to complete this work. The selected vendors who are awarded via a procurement process will also be involved with the permitting process. The vendors will then be responsible for the installation of the following based on their area of specialty: WINDOWS & DOORS • Provide NOA plans and obtain permitting • Engineer and assess design for appropriate wind pressure values for windows and doors • Remove 25 windows • Install 25 new storefront aluminum frames with fixed impact glass windows • Remove 9 doors • Install 9 new doors with fixed impact resistant glass • Apply molding, patch interior drywall and sills, and repair exterior stucco walls and touch up paint ROOFING • Provide design and engineering specifications, and obtain permitting • Prepare site and facility for work • Remove 11,475 sq. ft. of existing single roofing • Install new wind speed rated metal roof, flashing, vents and gutters GENERATOR • Generators (1 - 200kW/300 Amp) • Transfer Switch/Electrical connections • Concrete pads • Fuel tanks and bases • Testing/Training Proposed contractors have not been identified at this time; however, the County will go through the formal public solicitation process, an evaluation of the respondents' qualifications and experience, and lowest reasonable bid. Her professional experience and accomplishments include: capital project management for facility maintenance and vertical construction; internal controls of financial and fee policies for Collier County Government Zoning Division; negotiation of multiple fire district merger plans to support $6 billion in sustainable infrastructure; PM consultant for $100 million medical companies Singular Medical Technology and United Health Centers; created unified brand marketing and messaging for six departments to maximize program growth revenues by $7.5 million; and six years as President of Communications & Business Development for DREAMFly Marketing, LLC. This building — which is over 40 years old - has served in the past a public safety purpose for the local community. This region has seen many hurricanes and wildfires; this structure often operated as Post Disaster Recovery Center, where elderly and other residents had the opportunity to meet and interact with FEMA, Red Cross, and Salvation Army representatives. The structure is 40 years old; by hardening the integrity of the building envelope, and by making it stronger, the vulnerability of the population residing in this area could be greatly diminished, as well as the loss of civic resources. The disruption of essential public recovery services necessary after an emergency would be drastically reduced; the same can be said for negative economic impacts to businesses, and losses of cultural resources. Having a strong building that could safely and reliably host Post Disaster Recovery activities after an emergency event, has become an essential need for the residents in this area, especially for the elderly. To have a Disaster Recovery Center the population can count on, for the services mentioned above, but also for the distribution of water, meals, tarps and first necessity items would dramatically improve the safety of the Golden Gate area residents. The hardening of the building will consist of the following: replacement of the existing trusses and the installation of a new roof, replacement of all existing doors, louvers and windows with impact rated elements, wall hardening and building exterior retrofit achieved by using hazard resistant materials in accordance with national safety standards, drainage management, relocation of essential mechanical and electrical components above flood elevation, and the installation of a generator. The generator will assure power is provided to the building when the electrical grid is failing. Internet and phone lines must function. In addition, the existing lift station will be connected to the , an emergency secondary water supply will be connected to the building and a new septic tank installed adjacent to the hardened building will guarantee a proper functioning of the plumbing system throughout the emergency. The hardening will be in strict compliance with the current Florida Building Code. The following risks will be mitigated: the building will be protected against wind and flood damage, and loss of electrical power. Collier County will organize a public solicitation for this project open to all qualifiers holding a valid Florida Contractor License in accordance with Collier County procurement policy, as well as all Federal and State Laws and Regulations. The project will be awarded to the most responsive, most qualified and most competitive responder and will abide by all CBDG-MIT criteria. The project will be managed by Collier County Facilities Management Division. The ultimate outcome is to provide to the Community a building that has been sufficiently hardened to safely host uninterrupted support and aid activities to assist the population immediately after any emergency that may arise: hurricanes, tropical cyclones, severe storms, flooding, wildfires, cold snaps, etc. The cost of the project was determined by examining similarly hardened structures of comparable size and scope. Estimates are based on the current Market price and historical data. After completion, the maintenance of this facility will be completed by Collier County Facilities Management Division. The maintenance will be conducted through the County's work order system, City Work. Payment for maintenance will be funded through Collier County funds. This work should protect the building enough so that a reconstruction triggered by natural disasters will not be necessary. The Golden Gate Senior Center is over 40 years old. While some building elements — like the roof shingles and the roof underlayment - have already been replaced because damaged by hurricanes, other major building components are still very vulnerable. By implementing a complete "360 degree" building hardening, the resiliency of the structure will increase dramatically, and the potential impact of natural disasters and associated damages will be greatly reduced. If this project will not be completed, the population will not be able to benefit from much needed services that only a well -designed and organized Post Disaster Service Center can offer. Having a safe Post Disaster Service Center in this area is extremely important for the Golden Gate Community, which includes many elderlies. Community resilience will be enhanced not only in times of needs, but also in times when crisis do not take place. Educational, outreach, research and development programs can be run in the building to improve the knowledge and awareness among the citizens and industry about hazards they may face, and mitigation alternatives that can reduce vulnerabilities for the community. The project is benefitting the following lifelines: safety and security, food water and shelter, health and medical, energy (power & fuel), and communications. His background encompasses many facets of public administration with extensive experience in the areas of public works, public services, facilities management, and both contract and project management. He has assisted with the design, development, construction, and maintenance of multiple facilities in excess of 500,000 square feet. Prior to joining Collier County, Damon held the position of Director of Facilities Construction and Management for Lee County. He has a Bachelor's Degree in Public Administration and a Master's Degree in Administration and Human Resource Development. Ayoub AI-Bahou is a Licensed Principal Project Manager for the Facilities Management Division for Collier County Government. He has a Bachelor's Degree in Civil Engineering from Louisiana Tech University and is a licensed Professional Engineer and Green Associate LEED certified. Prior to joining Collier County, Ayoub had a 20 year career with the Lee County Department of Facilities Construction and Management and was responsible for completing many notable projects including the Lee Tran Administration, Operation & Maintenance Facility, Pine Ridge Government Complex, and the Lee County Sheriff's Office Forensic Center. Ayoub is additionally a member of various organizations including the American Society of Civil Engineers, the Florida Engineering Society, the National Society of Professional Engineers, and is a volunteering mentor for FGCU senior students in Civil Engineering. Healthcare Network, as a patient centered medical home, is the largest primary care provider in the area and serves 3 out of 20 people, 6 out of 10 children, and 4 out of 5 people living in poverty. In 2019, we provided more than $4.9M in uncompensated healthcare to children and their families. Fifty-eight percent of Healthcare Network's patients are under the Federal Poverty Level (FPL) and 71 % are under 200% of FPL, qualifying for our sliding fee discounts. Sixty-one percent are Hispanic and 14% are black. Thirty-nine percent of our adult patients are uninsured, as Florida has not expanded Medicaid to adults. Chronic diseases have an impact in Collier County and many of our patients have multiple comorbid conditions such as diabetes, hypertension, obesity, cardiovascular disease, chronic pain, and other ailments. These conditions can lead to and exacerbate high blood pressure, kidney failure, neuropathy, blindness, heart attack, stroke, and poor mental health. These conditions cause further suffering, increased and avoidable health care utilization, disability, and shortened lifespan. In 2018, HCN treated a total of 3,342 patients with diabetes, 2,809 patients with asthma, 5,281 patients with hypertension, 866 patients with heart disease, 14,935 patients with a diagnosis of being overweight or obese, and 9,567 patients with a mental health diagnosis. The proposed project will protect the assets inside the structure and allow patients to see their primary health provider soon after an event. With completion of the hardening of the project the potential for heat and water damage to many of the items in at the three different locations will be greatly reduced. While the primary purpose of Healthcare Network is to provide primary care to the community throughout the year, the facilities will play a critical role in the recovery of the community from disaster. The hardening of the structures with windows and doors means that the facilities can better withstand a wind event and the County can count on using the facility immediately after a disaster. The proposed project will be managed by the Facilities Department of Healthcare Network under the direction of an as yet to be identified project manager in coordination with the Collier County. Execution and completion of the project will be through a formal bid process, an evaluation of the respondent's qualifications and experience, and lowest responsible and reasonable bid. The proposed hardening project will mitigate water damage caused by intrusion through non -impact resistant windows and doors. Estimated costs of the hardening of the Healthcare Networks, windows and doors is from a quote by SafeZone, LLC a local contractor. If the project is funded Healthcare Network will obtain 3 formal bids from local contractors. Annual estimated costs for the maintenance of the wind retrofit doors and windows of the Healthcare Network's structures will be part of Healthcare Network's annual operating budget. Facilities Division, estimated annual costs for caulk, power washing, cleaning and basic repairs is $4,000. Page 12 Community Development Block Grant - Mitigation (CDBG-MIT) — Golden Gate Senior Center 00*7 C014"ty Community Development Block Grant - Mitigation (CDBG-MIT) — Healthcare Network Naples Scattered Sites Capacity Plan: Healthcare Network plays a critical role in the resiliency of the community. Numerous agencies and community partners look to Healthcare Network, during, and after emergency events to provide valuable information, resources, and access to services for resiliency and recovery efforts. Specific stakeholders beyond the low to moderate income residents of the Collier County are: Federal and State Agencies (including FEMA) Collier County Government and Constitutional Offices (Including Bureau of Emergency Services) Naples Chamber of Commerce Local Business Leaders Collier County Public Schools Redlands Christian Migrant Association (RCMA) Local Non -Profit Organizations The project will consist of retrofitting the facility with 66 replacement windows and doors. The scope of work for the project is to complete final design. Permitting, along with several vendors/contractors to complete the project elements. The design and engineering of the project will be limited in scope. The primary emphasis for the plans will be the windows and door. However, final plans will be required to obtain permits and to ensure that the installation of the project elements are done effectively and with no negative impact to the environment. WINDOWS & DOORS • Provide NOA plans and obtain permitting • Remove 66 existing windows and doors • Install 66 new code compliant insulated/impact glazing system • Apply molding, patch interior drywall and sills, and repair exterior stucco walls and touch up paint Proposed contractors have not been identified at this time; however, Healthcare Network will go through the formal bid process, an evaluation of the respondents' qualifications and experience, and lowest responsible and reasonable bid. Project staff will include the following: In 2006, Rod Stitt, Director of Facilities, moved to Southwest Florida and was hired to work in purchasing by the Finance Department at the Healthcare Network. Over the next 14 years, his roles grew within the finance department as he worked as a purchasing agent, purchasing manager, and then director of facilities. In 2009, Rod was named to the role of Director of Facilities, and in this role Rod oversees all Healthcare Network buildings, purchasing staff, maintenance and housekeeping staff, works with all outside landlords, HVAC contracts, generator contracts, deals with all construction, equipment maintenance contracts, landscaping contracts and oversees all vehicles and vehicle maintenance. As the Director of Facilities, Rod works closely with the Operational Directors to develop and maintain accreditations with HRSA and AAAHC. A native of North Fort Myers, Tami Raznoff left Southwest Florida to attend Haywood Community College in North Carolina, earning an associate's degree in computer programing. In 1999, Tami returned to Southwest Florida and was hired to work in accounts payable by the Finance Department at the Healthcare Network. Over the next 20 years, her roles grew within the finance department as she worked in accounts receivable, payroll, as a purchasing agent, a grants management accountant, and then as the general accounting manager, controller, and senior director or finance. In 2012, Tami received her Bachelor of Science in Accounting from Florida Gulf Coast University. In 2019, she was named to the role of CFO for the Healthcare Network, and in this executive role Tami oversees all financial operations for the organization including finance, billing, reporting, coding, credentialing and facilities. As the CFO, works closely with the CEO, board of directors, organizational leaders and employees to develop financial goals to protect the organization's assets and investments. With 25 years of experience in accounting, Tami is currently pursuing a Master's in Business Administration from Florida Gulf Coast University and plans on taking the Certified Public Accountant Exam when she finishes her MBA. Throughout her career at Healthcare Network, Tami has dedicated herself to the mission of providing quality healthcare accessible to all and as such often volunteers herself to leadership roles in support of the network's initiatives. Such efforts will reduce the risk and cost of repair or rebuilding of exterior structural damage and/or interior structural, asset, and resources damage arising from a major natural event. Should this project not be funded and completed, Healthcare Network risks increased vulnerability to natural and human -caused threats and hazards. The duration of these adverse consequences jeopardizes the resiliency and recovery efforts of the community. The hardening at the 3 different Healthcare Network facilities in Naples benefit the Safety and Security Lifeline by allowing the residents of Collier County to seek medical care during a disaster. Healthcare Network was established in Collier County in 1977 to provide safety -net care for those in the community that fall under the category of the greatest need. Originally instituted to help the uninsured farmworker population obtain equality in healthcare, Healthcare Network has since flourished throughout Collier County. Healthcare Network service area, Collier County is 2,304.93 square miles, the second largest county in the state with a population of over 367,000. Healthcare Network, as the only Federally Qualified Health Center (FQHC) in Collier County, provided 202,061 visits to 49,943 patients in 2019. Thirty percent of HCN's patients live in Immokalee; ZIP code 34142 & 34143, which has the highest level of uninsured residents in Collier County. Healthcare Network has a very diverse workforce. 62% of the 368 staff members are Hispanic and 13% are Black, which is representative of the service area. (CHS PAYLOCITY Report, March 2020). Most of the staff at Healthcare Network that works directly with patients are bilingual in either Spanish or Haitian Creole. Healthcare Network employs translators and contacts with Certified Languages International (CLI) for interpretation services via telephone invarious languages. Our printed materials for patients are available in English, Spanish and Haitian Creole. Collier County, Florida has been designated a Medically Underserved Area (MUA). MUAs are areas or populations designated by HRSA as having too few primary care providers. Additionally, Healthcare Network's Federal Health Professional Shortage Area's (HPSA) designations were updated by HRSA in August, 2019, reflecting a shortage of health care providers (primary care, dental and mental health) as well as the existence of barriers to accessing care including lack of public transportation, travel time and distance to the next source of undesignated care and high poverty. The HPSA designation scores counties between one and twenty-six, with the higher scores indicating higher levels of need. Healthcare Network's high scores of 21 for primary medical, 25 for dental, and 23 for mental healthcare indicates severe shortages. Healthcare Network has created Collier County's first integrated care system, an organized coordination of primary and behavioral healthcare. We value our nursing -driven programs and integrate behavioral health (BH) services in our clinics. We have a primary focus on population -health and prevention, screening, brief intervention in the primary care setting, and referral and coordination to specialty mental health services. Unlike the traditional model of dental, medical, and behavioral services performed by different providers in different settings, integrated healthcare brings dentists, physicians, nurse -practitioners, nurses, pharmacists, and psychologists together working as a team. Most aspects of a patient's wellbeing can be addressed, often in one collaborative visit, as opposed to having to schedule two or more separate visits. Patients with hypertension, diabetes, heart disease, and obesity were the most severe chronic diseases in 2019. Age, race, ethnicity and geographical location were identified as common factors in these chronically diseased patients. Healthcare Network's Hispanic and Haitian patients with these diseases are most vulnerable. Our high risk patients are the most vulnerable during the pandemic and we do not want to take the chance of them contracting Covid-19 by coming to an on -site visit. Healthcare Network just started providing telehealth visits for our Behavioral Health visits and will enhance our telehealth program to include all our providers Healthcare Network's program is unique because we have extensive electronic medical records of the Hispanic and Haitian patients with these chronic illnesses. Our care team staff is bilingual in Spanish/English or Creole/English. We will assist with the technology at the patient's home and our behavioral health team members can help to reduce anxiety. Early detection of changes in a patient's condition is the primary benefit of home tele monitoring. Long-term potential impacts include ensuring that all community resources and community programs are being maximized with improved care coordination, fewer specialist referrals, less emergency usage and reduced health expenditures supporting the goal of the patient's improved health. Fether Medical Center Project Description: Collier Health Services, Inc. dba Healthcare Network Marion E. Fether Medical Center 1454 Madison Ave, Immokalee, FL Marion E. Fether Medical Center is recognized by the community as the primary location to receive quality healthcare in Immokalee. The 40,750 square foot public facility is surrounded by parks, public schools, County offices and is easily accessible through a major roadway. Collier Health Services, Inc. dba Healthcare Network is the only Federally Qualified Health Center (FQHC) in Collier County, we provided 202,061 visits to 49,943 patients in 2019. Healthcare Network, as a patient centered medical home, is the largest primary care provider in the area and serves 3 out of 20 people, 6 out of 10 children, and 4 out of 5 people living in poverty. In 2019, we provided more than $4.9M in uncompensated healthcare to children and their families. Fifty-eight percent of Healthcare Network's patients are under the Federal Poverty Level (FPL) and 71 % are under 200% of FPL, qualifying for our sliding fee discounts. Sixty-one percent are Hispanic and 14% are black. Thirty-nine percent of our adult patients are uninsured, as Florida has not expanded Medicaid to adults. Chronic diseases have an impact in Collier County and many of our patients have multiple comorbid conditions such as diabetes, hypertension, obesity, cardiovascular disease, chronic pain, and other ailments. These conditions can lead to and exacerbate high blood pressure, kidney failure, neuropathy, blindness, heart attack, stroke, and poor mental health. These conditions cause further suffering, increased and avoidable health care utilization, disability, and shortened lifespan. In 2018, HCN treated a total of 3,342 patients with diabetes, 2,809 patients with asthma, 5,281 patients with hypertension, 866 patients with heart disease, 14,935 patients with a diagnosis of being overweight or obese, and 9,567 patients with a mental health diagnosis. The proposed project will protect the assets inside the structure and allow patients to see their primary health provider soon after an event. With completion of the hardening of the project the potential for heat and water damage to many of the items in at the three different locations will be greatly reduced. While the primary purpose of Healthcare Network is to provide primary care to the community throughout the year, the facilities will play a critical role in the recovery of the community from disaster. The hardening of the structures with windows and doors means that the facilities can better withstand a wind event and the County can count on using the facility immediately after a disaster. The proposed project will be managed by the Facilities Department of Healthcare Network under the direction of an as yet to be identified project manager in coordination with the Collier County. Execution and completion of the project will be through a formal bid process, an evaluation of the respondent's qualifications and experience, and lowest responsible and reasonable bid. The proposed hardening project will mitigate water damage caused by intrusion through non -impact resistant windows and doors. Estimated costs of the hardening of the Healthcare Networks, windows and doors is from a quote by SafeZone, LLC a local contractor. If the project is funded Healthcare Network will obtain 3 formal bids from local contractors. Annual estimated costs for the maintenance of the wind retrofit doors and windows of the Healthcare Network's structures will be part of Healthcare Network's annual operating budget. Fether Medical Center Community Value Narrative: This project would harden all windows and doors throughout the building bringing them up to current code for wind and impact resistance. Such efforts will greatly reduce the center's vulnerability to exterior and / or interior structural damage, as well as minimize asset and resource damage arising from a major natural event. Should this project not be funded and completed, Healthcare Network risks increased vulnerability to natural and human -caused threats and hazards. The Marion E. Fether Medical Center is Healthcare Network's corporate headquarters. If this major community health center were to sustain significant damage the adverse consequences could jeopardize the resiliency and recovery efforts of the community. After hurricane Irma Healthcare Network was the first to respond in the community of Immokalee with our Mobile Units. The hardening at The Marion E. Fether Medical Center would directly benefit the Safety and Security Lifeline by allowing the residents of Immokalee to seek medical care during a disaster. Healthcare Network was established in Collier County in 1977 to provide safety - net care for those in the community that fall under the category of the greatest need. Originally instituted to help the uninsured farmworker population obtain equality in healthcare, Healthcare Network has since flourished throughout Collier County. Healthcare Network service area, Collier County is 2,304.93 square miles, the second largest county in the state with a population of over 367,000. Most of the staff at Healthcare Network that works directly with patients are bilingual in either Spanish or Haitian Creole. Healthcare Network employs translators and contacts with Certified Languages International (CLI) for interpretation services via telephone in various languages. Our printed materials for patients are available in English, Spanish and Haitian Creole. Collier County, Florida has been designated a Medically Underserved Area (MUA). MUAs are areas or populations designated by HRSA as having too few primary care providers. Additionally, Healthcare Network's Federal Health Professional Shortage Area's (HPSA) designations were updated by HRSA in August, 2019, reflecting a shortage of health care providers (primary care, dental and mental health) as well as the existence of barriers to accessing care including lack of public transportation, travel time and distance to the next source of undesignated care and high poverty. The HPSA designation scores counties between one and twenty-six, with the higher scores indicating higher levels of need. Healthcare Network's high scores of 21 for primary medical, 25 for dental, and 23 for mental healthcare indicates severe shortages. Healthcare Network has created Collier County's first integrated care system, an organized coordination of primary and behavioral healthcare. We value our nursing -driven programs and integrate behavioral health (BH) services in our clinics. We have a primary focus on population -health and prevention, screening, brief intervention in the primary care setting, and referral and coordination to specialty mental health services. Unlike the traditional model of dental, medical, and behavioral services performed by different providers in different settings, integrated healthcare brings dentists, physicians, nurse -practitioners, nurses, pharmacists, and psychologists together working as a team. Most aspects of a patient's wellbeing can be addressed, often in one collaborative visit, as opposed to having to schedule two or more separate visits. Patients with hypertension, diabetes, heart disease, and obesity were the most severe chronic diseases in 2019. Age, race, ethnicity and geographical location were identified as common factors in these chronically diseased patients. Healthcare Network's Hispanic and Haitian patients with these diseases are most vulnerable. Our high risk patients are the most vulnerable during the pandemic and we do not want to take the chance of them contracting Covid-19 by coming to an on -site visit. Healthcare Network just started providing telehealth visits for our Behavioral Health visits and will enhance our telehealth program to include all our providers Healthcare Network's program is unique because we have extensive electronic medical records of the Hispanic and Haitian patients with these chronic illnesses. Our care team staff is bilingual in Spanish/English or Creole/English. We will assist with the technology at the patient's home and our behavioral health team members can help to reduce anxiety. Early detection of changes in a patient's condition is the primary benefit of home tele monitoring. Long-term potential impacts include ensuring that all community resources and community programs are being maximized with improved care coordination, fewer specialist referrals, less emergency usage and reduced health expenditures supporting the goal of the patient's improved health. Numerous agencies and community partners look to Healthcare Network, during, and after emergency events to provide valuable information, resources, and access to services for resiliency and recovery efforts. The scope of work for the project is to complete final design. Permitting, along with several vendors/contractors to complete the project elements. The design and engineering of the project will be limited in scope. The primary emphasis for the plans will be the windows and door. However, final plans will be required to obtain permits and to ensure that the installation of the project elements are done effectively and with no negative impact to the environment. Project staff will include the following: In 2006, Rod Stitt, Director of Facilities, moved to Southwest Florida and was hired to work in purchasing by the Finance Department at the Healthcare Network. Over the next 14 years, his roles grew within the finance department as he worked as a purchasing agent, purchasing manager, and then director of facilities. In 2009, Rod was named to the role of Director of Facilities, and in this role Rod oversees all Healthcare Network buildings, purchasing staff, maintenance and housekeeping staff, works with all outside landlords, HVAC contracts, generator contracts, deals with all construction, equipment maintenance contracts, landscaping contracts and oversees all vehicles and vehicle maintenance. As the Director of Facilities, Rod works closely with the Operational Directors to develop and maintain accreditations with HRSA and AAAHC. A native of North Fort Myers, Tami Raznoff left Southwest Florida to attend Haywood Community College in North Carolina, earning an associate's degree in computer programing. In 1999, Tami returned to Southwest Florida and was hired to work in accounts payable by the Finance Department at the Healthcare Network. Over the next 20 years, her roles grew within the finance department as she worked in accounts receivable, payroll, as a purchasing agent, a grants management accountant, and then as the general accounting manager, controller, and senior director or finance. In 2012, Tami received her Bachelor of Science in Accounting from Florida Gulf Coast University. In 2019, she was named to the role of CFO for the Healthcare Network, and in this executive role Tami oversees all financial operations for the organization including finance, billing, reporting, coding, credentialing and facilities. As the CFO, works closely with the CEO, board of directors, organizational leaders and employees to develop financial goals to protect the organization's assets and investments. With 25 years of experience in accounting, Tami is currently pursuing a Master's in Business Administration from Florida Gulf Coast University and plans on taking the Certified Public Accountant Exam when she finishes her MBA. Powered by Qualtrics Z Co�er Co-Unty Community Development Block Grant - Mitigation (CDBG-MIT) — Immokalee Sports Complex Hardening Proposal Project Description: The purpose of this project is to provide a safe environment for the citizens of Immokalee through hardening of existing facility used before, during, and after natural disasters. Specifically, the project seeks to harden existing infrastructure by replacing the roof of the facility with upgraded hurricane/wind resistant materials, replace an existing covered walkway that was installed 30 years ago and currently floods areas between the gym location and fitness center, which is used for sheltering, and install a new HVAC and ventilation system for existing fitness center, administration, and gym, and provide for renovation of stucco of gym, providing dry proofing with paint and sealant. Recipients of this project are considered citizens of Immokalee with 100% of the area being considered eligible to receive benefits as low and moderate income. The Collier County Parks and Recreation Division will be working with its sister Division Community Health Services (CHS) to oversee the grant requirements related to the project. CHS will ensure grant compliance is maintained and provide administrative oversight of the project. The Facilities Management Division will be responsible for the oversight of assessment, design, and permitting related to the construction/renovation of the project. Currently, Immokalee Sports Complex is a 25-year structure that provides local recreation for residents of Immokalee. The facility has a public pool, fitness center, and gymnasium. The facility has historically been used in times of natural disaster to provide a cooling center to area residents, as well as serving as a location for housing individuals displaced from their homes due to natural disaster (hurricane). The facility has been the recipient of HGMP funds after Hurricane Charlie and was retrofitted with hurricane shutters to protect the building during storms. Recently, the project received Hazard Mitigation Grant Funding (HGMP) for permanent generators to support the facility during natural disaster. The anticipated outcome is to harden the existing facility, provide increase ventilation within the fitness center that is also used for shelter, remove and replace infrastructure related to canopy that join the fitness center/gym area and further strengthen its ability to sustain natural disasters and be a place for relief of area citizens. Cost estimates were derived from Facilities Management, who employee professional engineers who construct cost estimates based on current pricing for construction. Did you complete a similar project? Roof replacement and HVAC has been completed and pricing consistent with the figures provided are available for comparison. Maintenance of the facility will be completed by the Facilities Management Division. Facilities Management is responsible for the overall maintenance of all Parks and Recreation, as well as all other county facilities. The Collier County Board of County Commissioners provides appropriations for the maintenance of all county facilities. 00*7 C014"ty Community Development Block Grant - Mitigation (CDBG-MIT) — Immokalee Sports Complex Hardening Proposal Community Value Narrative: Historically, the Immokalee Sports Complex has served as a community resource for the citizens of Immokalee. The park grounds are home for a variety of community events and the park and facilities is well known amongst the citizenry, who view the park as an extension of their homes due to the activity and familiarity. In times of need, the facility has served to shelter those displaced from their homes. Most recently, during Hurricane Irma, the facility served those who were displaced and originally sheltered in the local high school. The Immokalee Sports Complex became a step-down unit from the local high school, so the high school could reopen to operate as a school. This role allowed for the Division to assist in helping the schools reopen while at the same time continuing to provide for the need of those displaced. Not obligating the school district to continue to provide for longer than needed sheltering, allowing for the schools to re- open, and allowing for the continued sheltering of individuals has an incalculable, but important cost savings. In addition, providing for shelter during times of emergency is yet another cost savings for the community. Current conditions of the facility are hampered due to wind and water intrusion from previous storms. While the current roofing system is enough for normal weather conditions, hurricane force winds has forced water into the building envelope causing damage and has debilitated the existing infrastructure of the building. Repeated exposure to these conditions, will worsen the infrastructure of the facility and keep it from being used to its current ability to be a resource during times of emergency. How does this project if not completed pose a significant risk to public health and safety? The Immokalee Sports Complex serves as a hub for the Immokalee community and has historically been available for use during times of emergency. In addition, the facility also provides back up for other resources provided by the Collier County School District. By not hardening or keeping this asset available during times of emergency need, the School District will be overburdened during crisis, delaying the recovery of the community for needing to support emergency activities before, during, and after hurricane and other natural disasters. Also, the School District will be delayed in re -opening, which schools re -opening after a catastrophic event are vital to the reestablishment of a community after such events. Health and Wellness and Safety and Security, as well as, energy resilience with the generators are the lifelines this project benefits. This project will benefit the lifelines identified above related to materials and supplies related to critical hardening of the facility, dry proofing to prevent water intrusion by waterproofing exterior walls and panels and providing increased ventilation. A portion of the facility was once part of the technical campus associated with the Collier County School District. Finally, the project seeks to enhance ventilation within the fitness center building to provide comfort to those who are sheltered in this area during times of need by providing a new HVAC system for the fitness center, gym, and administration area. Contractors will be solicited from an approved contract by the Board of County Commissioners, who have been selected in their provision of service in accordance with all federal, state, and local guidelines. Five (5) projects as outlined below are part of the application. Public facility hardening at three (3) locations in Healthcare Network of SW Naples including medical and dental facilities. $ 475,965 FL Naples Replacement of 66 windows and doors with code compliant insulated/impact windows and doors. The grant amount of $6,445,174 is 100% federally funded and requires no match. An additional $175,000 in local contribution will provide for furniture, fixtures, and equipment as part of the Golden Gate Senior Center hardening project. These items are not eligible under the CDBG-MIT grant program. Local funds are available within the County- wide Capital Project Fund 301 under the Golden Gate Senior Center Rehab Project and earmarked within the FY21 proposed budget supported by a transfer from General Fund (001). The Florida Department of Economic Opportunity announced funding availability on April 15, 2020 with applications due no later than June 30, 2020.
