A RESOLUTION APPOINTING WILLlA;\J C.
SHAFER TO TilE BOARD OF DIRECTORS
OF TilE HEALTH PLANNING COUNCIL OF
SOUTHWEST FLORIDA, INC.
WHEREAS, the Board of County Commissioner received notification from the Ilcalth
Planning Council of South\vest Florida. Inc,. that there is currenc)' a vacancy lo he Iilled hy the
Collier County Commission; and
WHEREAS. a press release was issued by council soliciting applications for appointment;
and
WHEREAS. the Executive Director of the Health Planning Council of Southwest Florida,
rnc.. has submitted its letter of recommendation to the Board ofCounty Commissioners,
NOW, THEREFORE, BE IT RESOLVED BY THE BOARD OF COUNTY
COMMISSIONERS OF COLLIER. COUNTY, FLORIDA. that William C. Shafer is hereby
appointed to the Board of Directors of the Health Planning Council of Southwest Florida, Inc., to
fulfill the remainder of the vacant tem" said term to expire on September JO, 1999.
This Resolution adopted after motion, second and majority vote.
