Limited Liability Company Affidavit
Before me, the undersigned authority, personally appeared Chetan Patel, ("Affiant') who being by me first duly
sworn, on oath deposes and says that:
1. He is currently the Manager of LAXMI of Naples, LLC, a Florida limited liability company
("LLC")
2. Said LLC is currently in existence under valid articles of organization and regulations and has
not been terminated or dissolved.
3. The following parties are all of the members/managers of said LLC:

4. Affiant is authorized by the articles of organization or operating agreement of said LLC to
execute deeds and mortgages on behalf of said LLC, and all necessary consents have been
obtained.
5. The LLC is not currently a debtor in any bankruptcy proceeding, and this conveyance or
mortgage is in the ordinary course of business.
6. This affidavit is given to induce Commonwealth Land Title Insurance Company to issue its title
policy insuring the contemplated transaction.
7. Affiant further states that he is familiar with the nature of an oath and with the penalties as
provided by the laws of the State of Florida for falsely swearing to statements made in an
instrument of this nature. Affiant further certifie has fully read this affidavit and
understands its contents.
