EXECUTIVE SUMMARY
Recommendation to approve a Second Amendment to Agreement No. 15-6474R, “Medical Director
for County Occupational Health Services (Part A) and Employment Physicals & Drug Testing
(Part B),” with Advance Medical Center, LLC.
OBJECTIVE: To extend an Agreement for an additional five (5) years with updated pricing for the
continuation of medical oversight services for the County’s Occupational Health Program.
CONSIDERATION: On February 9, 2016 (Agenda Item 16.E.1), the County entered i nto Agreement
No. 15-6474R (the “Agreement”) with Advanced Medical of Naples, LLC (“Provider”) to provide
medical services and oversight physician for the Occupational Health staff. On October 15, 2018, the
County administratively approved the First Amendment to the Agreement to include two additional
testing categories to Exhibit B-Price, Part B.
The Agreement is set to expire on February 8, 2020. Staff reviewed various local options for obtaining
Occupational Health services in Collier County. Advance Medical Center, LLC, was the only provider in
Collier County who possessed the credentials to administer a full service program. Advance Medical
specializes in Occupational Medicine. They offer services 7 days a week with extended office hours,
diagnostic testing for Occupational Medicine, onsite drug screening and an in-house Medical Review
Officer. Staff determined that the only comparable option would require that employees travel to Lee
County, resulting in increased program cost; loss of productivity; longer wait times to finalize post offer
hiring; and hardship to employees/applicants and the operation.
The attached Second Amendment will extend the Agreement for an additional five (5) years, commencing
on February 09, 2020 and ending on February 8, 2025, and incorporate an updated Pricing exhibit. Except
as modified through this Amendment, all other terms and conditions of the Agreement shall remain the
same.
FISCAL IMPACT: Funds are budgeted annually in the Workers’ Compensation (Fund 518) budget
and department budgets for required physicals and drug tests. Total funds expended are dependent upon
the need for services. The estimated annual expenditure for FY20, based upon the proposed rates and
estimated volume of service is $89,449.07.
LEGAL CONSIDERATIONS: This item is approved as to form and legality and requires majority vote
for Board approval. -SRT
RECOMMENDATION: That the Board approves the Second Amendment to Agreement No. 15-6474R
and authorizes the Chairman to sign the attached Amendment.

Good Morning,
In reference to the aforementioned agreement Risk Management would like to point out that
Section 12 E on the contract is a duplicate of Section 12 D; for Medical Professional Liability and
Medical Malpractice are the same coverage. During the BID process both items were checked by
mistake; but the only coverage that is required is Medical Professional Liability with limits of
$250,000 per occurrence and $750,000 aggregate. These limits are consistent with the limit carried
under this agreement in the past. Therefore, the Certificate of Insurance providing proof of
coverage for the Medical Professional Liability with a limit of $250,000 is in compliance.
