by the Office of the County Attorney.
4. All handwritten strike-through and revisions have been initialed by the County Attorney's N/A
Office and all other parties except the BCC Chairman and the Clerk to the Board
5. The Chairman's signature line date has been entered as the date of BCC approval of the JAK
document or the final negotiated contract date whichever is applicable.
6. "Sign here"tabs are placed on the appropriate pages indicating where the Chairman's JAK
signature and initials are required.
7. In most cases(some contracts are an exception),the original document and this routing slip N/A
should be provided to the County Attorney Office at the time the item is input into SIRE.
Some documents are time sensitive and require forwarding to Tallahassee within a certain
time frame or the BCC's actions are nullified. Be aware of your deadlines!
8. The document was approved by the BCC on 12/10/19 and all changes made during
the meeting have been incorporated in the attached document. The County JAK
Attorney's Office has reviewed the changes, if applicable.

RESOLUTION NO. 2019 - 2 4 5
A RESOLUTION OF THE BOARD OF COUNTY COMMISSIONERS OF
COLLIER COUNTY, FLORIDA, APPOINTING JEFFREY LUTZ TO THE
COLLIER COUNTY INFRASTRUCTURE SURTAX CITIZEN
OVERSIGHT COMMITTEE.
WHEREAS, on April 24, 2018, the Board of County Commissioners (Board) adopted
Ordinance No. 2018-21, which established the Collier County Infrastructure Surtax Citizen
Oversight Committee (Committee) as an ad hoc advisory and reporting body to the Board to
provide citizen review of the expenditure of the Surtax proceeds against the ballot referendum
approved by the voters and the Ordinance; and
WHEREAS, Ordinance No. 2018-21, as amended, provides for membership to be
composed of seven regular members (including one member from each of the five Commission
Districts and two at-large members) and two alternate members; and
WHEREAS, the seat representing District 1 is currently vacant; and
WHEREAS, the Board has provided public notice soliciting applications from interested
parties; and
WHEREAS, the Commissioner for District 1 has provided her nomination for
appointment.
NOW, THEREFORE, BE IT RESOLVED BY THE BOARD OF COUNTY
COMMISSIONERS OF COLLIER COUNTY, FLORIDA, that JEFFREY LUTZ is hereby
appointed to the Collier County Infrastructure Surtax Citizen Oversight Committee, to serve the
remainder of a vacant term expiring on January 12, 2021.
THIS RESOLUTION ADOPTED upon majority vote on the 10th day of December, 2019.
