EXECUTIVE SUMMARY
Recommendation to approve the Second Amendment to Agreement # 14-6293, “Collier County
Onsite Medical Clinic,” with Millennium Physician Group, LLC, at an estimated first year
expenditure of $693,367.
OBJECTIVE: To continue providing onsite medical and limited pharmacy services to employees and
dependents covered under the Collier County Group Health Plan for the purpose of promoting health,
wellness, and productivity.
CONSIDERATIONS: The Risk Management Division administers a partially self-funded Group Health
Insurance Program for its employees, participating constitutional officer employees and their eligible
dependents. In 2006, the Board approved the creation of an onsite medical clinic to provide medical care
to employees and dependents covered under the Group Health Insurance Program. Since that time,
services provided through the clinic have expanded beyond acute care to include the management of
chronic conditions such as pre-diabetes and diabetes under the Smart Choice program; cardiac health
under the Heart Smart program; and weight management under the New You program. Services are
provided at the Main Campus location as well as a north clinic location.
On September 9, 2014 (Agenda Item 16.E.7), the County awarded Millennium Physicians Group, LLC
(“Millennium”) an initial five-year term contract with a five-year renewal option. Further, on October 10,
2017 (Agenda Item 16.E.2), the County approved the First Amendment to the Agreement providing for
the addition of limited pharmacy services through the MedCenter.
The initial contract term will end on November 13, 2019. In anticipation of the need for updated pricing
for the remaining five (5) years of the contract, staff requested pricing for years six (6) through ten (10).
The Second Amendment to the Agreement provides for the following changes:
1. Renewal of the Agreement for one additional five-year period to begin on November 14, 2019
and ending on November 13, 2024;
2. Replacing Exhibit A, Scope of Services in its entirety by removing expanded language for clinic
options 2, 3, and 4; and
3. Incorporating Exhibit B-1, pricing for the next five years with an effective date of November 14,
2019.
Services provided by Millennium have been satisfactory. Further, proposed pricing for the renewal is
substantially less than expected medical inflation over the renewal period. Staff recommends approval of
the renewal Amendment.
FISCAL IMPACT: The total estimated annual cost to provide primary care services in year one is
$693,367 based upon an estimated monthly enrollment of 2,236 employees. The annual cost in years two
through five will be based upon the contracted monthly rate per employee in Exhibit B-1 multiplied by
the monthly enrollment for the billing period. Sufficient funds have been budgeted in Fund 517, Group
Health and Life for this purpose.
GROWTH MANAGEMENT IMPACT: There is no growth management impact associated with this
item.
LEGAL CONSIDERATIONS: This item is approved as to form and legality and requires majority vote

EXECUTIVE SUMMARY
Recommendation to approve First Amendment to Agreement #14-6293 with Millennium Physician
Group, LLC to add limited pharmacy services through the MedCenter.
OBJECTIVE: To provide limited pharmacy services through the MedCenter for promoting health,
wellness, and productivity.
CONSIDERATIONS: The employee health clinic (a/k/a the MedCenter) was established in December
2006. The MedCenter is currently managed under an agreement with Millennium Physicians Group, LLC
(Board approved on September 9, 2014, Agenda Item 16.E.7). At its inception, the focus of services was
limited to acute and primary care services to members covered under the Collier County Group Health
Plan. However, with the implementation of the Invest in Your Health Program in 2009 as well as the
Board’s approval of Resolution #2017-67 to become certified as a Blue Zones employer, services have
expanded to include chronic disease management, medically monitored weight loss programs, a diabetes
management program, a tobacco cessation program and a cardiac health program.
To complement these services, the Risk Management Division is seeking approval from the Board to
permit limited pharmacy services through The MedCenter. These limited pharmacy services will include
commonly prescribed medications such as antibiotics and flu vaccines; however, narcotics will not be
included in the program. These medications will be maintained and tracked by The MedCenter staff and
the cost of these medications will be passed through to the Collier County Group Health Plan with no
mark up. It is the opinion of staff that this program will add value and enhance employee health and
productivity by allowing treatment to begin immediately (in the office); by eliminating an additional stop
at the pharmacy; by reducing sick leave usage; and by enabling the employee to return to work sooner.
By amending the agreement so that the current clinic services offered includes limited pharmacy services,
if the County elects to exercise any of the three existing enhanced clinic options under the agreement in
the future, those options would also include these services.
FISCAL IMPACT: The estimated annual cost of the program is $11,774. However, this amendment
does not add additional cost to the overall Group Health Plan. This amendment simply redirects payment
for these limited medications (at cost) to Millennium Physicians Group rather than to a local pharmacy.
There are sufficient funds in Fund 517, Group Health and Life Insurance to cover the cost of the program.
LEGAL CONSIDERATIONS: This item is approved as to form and legality, and requires majority

this action.
RECOMMENDATION: To approve the attached amendment to Agreement 14-6293, Onsite Medical
Clinic with Millennium Physicians Group, LLC as outlined above to provide limited pharmacy services
and authorize the Chair to sign the First Amendment.
