CERTIFICA TJON OF FINANICAL RESPONSIBILITY
The Collier County Water-Sewer District, a unit oflocal government of the State of Florida, hereby
certifies that it has unconditionally obligated itsclfto have the financial resources necessary to
close, plug, and abandon its Class I underground injection well and relatcd monitoring wells, as
required by Chapter 62-528 of the Florida Administrative Code. It is further understood that the
cost cstimated to conduct plugging and abandonment, established on December 24, 2008, shall be
reviewed on an annual basis and this obligation shall incorporate inflation costs. An annual
adjustment exceeding I () percent in anyone year shall require submission of an updated
certification form.
