RESOLUTION NO. 2002 - 299
RESOLUTION AUTHORIZING THE EXECUTION AND ACCEPTANCE OF
A MODIFICATION TO AGREEMENT #02-PR-37-09-21-01-001 BETWEEN
THE FLORIDA DEPARTMENT OF COMMUNITY AFFAIRS AND
COLLIER COUNTY, FLORIDA
WHEREAS, on September 11,2001, Collier County entered into Agreement #02-
PR-37-09-21-01-001 with the Florida Department of Community Affairs for funds in
the amount of $58,000 to be used for shelter enhancements at Golden Terrace and
Big Cypress Elementary Schools, and
WHEREAS, Collier County has entered into a contract for the shelter enhancements
necessary in order for Golden Terrace and Big Cypress Elementary Schools to meet
the American Red Cross 4496 standards for shelters, and
WHEREAS, there is a delay in shipment by a supplier of a final component required
to complete the shelter enhancements at Golden Terrace and Big Cypress Elementary
Schools, and
WHEREAS, Collier County has requested and the Florida Department of
Community Affairs has agreed to a modification to the Agreement extending the
completion date to September 30, 2002
NOW THEREFORE BE IT RESOLVED BY THE BOARD OF COUNTY
COMMISSIONERS OF COLLIER COUNTY, FLORIDA that the Board approve
Modification #1 extending the completion date to Agreement #02-PR-37-09-21-01-
001 between the State of Florida, Department of Community Affairs and Collier
County as presented to the Board for review, and hereby authorize the Board's
chairman to execute the modification on behalf of Collier County.
This Reso~tion adopted after motion, second, and majority vote in favor of adoption

MODIFICATION #1 TO AGREEMENT
WHEREAS, The State of Florida, Department of Community Affairs, with headquarters in
Tallahassee, Florida (hereinafter referred to as the "Department"), and Collier County (hereinafter referred to
as the "Recipient") have entered into a Agreement dated October 2, 2001, for the purpose of retrofitting
schools with window/door protection systems, performing engineering assessments and retrofitting roofs; and
WHEREAS, there is a delay in shipment by a supplier of a final component required to complete the
Golden Terrace and Big Cypress Elementary Schools.
NOW, THEREFORE, in consideration of the mutual promises herein contained, the parties agree to
modify the Agreement to read as follows:
3. PERIOD OF AGREEMENT
This Agreement shall begin upon execution by both parties and shall end September 30, 2002,
unless terminated earlier in accordance with the provisions of Paragraph 9 of this Agreement. Any
requests for reimbursement received after December l5, 2002, may, in the discretion of the
Department, not be reimbursed from this Agreement. Reimbursement requests shall not be
submitted by facsimile transmission.
All other terms and conditions of the Agreement inconsistent with this modification shall remain in
full force and effect.
IN WITNESS WHEREOF, the parties hereto have caused this modification to be executed by the
undersigned officials as of the date listed below.
