EXECUTIVE SUMMARY
Recommendation to approve and authorize the Chairman to sign amendment #3 between
Collier County Board of County Commissioners and the Area Agency on Aging of
Southwest Florida, Inc. and approve a budget amendment to reflect an increase of $25,000
in the Older Americans Act program.
OBJECTIVE: The execution of this amendment and budget amendment
recognize an increase in grant funding in the Services for Seniors program.

IS necessary to
CONSIDERATIONS: Collier County Services for Seniors provides in-home support services
to Collier County's frail elderly through the Older Americans Act program. An amendment to
increase OAA funding has been received which reflects an increase in the amount of $25,000 in
the Title C-l Congregate Meals program. The contract period for the OAA program is from
January I, 2009 through December 31,2009.
GROWTH MANAGEMENT: There is no growth management impact from this
recommendation.
FISCAL IMPACT: FY 2010 budget in the Older Americans Act cost center (155972) will
increase by $25,000. No additional matching funds are required.
LEGAL CONSIDERATIONS: The State requires that the County sign this amendment first.
This item has been reviewed and approved by the County Attorney's Oftice. This item is legally
sufficient for Board action.-CMG
RECOMMENDA TIONS: Staffrecommends that the Board of County Commissioners approve
and authorize the Chainnan to sign the amendment and approve the necessary budget
amendment.

Collier County Housinlland Human Servic~
TIDS CONTRACT is entered into between Area Agency on Aging for Southwest Florida,
Inc., hereinafter referred to as the "Agency", and Collier County Housinj! and HU.!!.lan
Services. hereinafter referred to as the "Vendor". This contract is subject to all provisions
contained in the CONTRACT executed between the Agency and the vendor, Contract No.
OAA 203.09, and its successor, incorporated herein by reference,
I. The purpose of this amendment is to revise the Attachment III, Exhibit-I and Budget
Summary, Attachment VII. The Collier County budget is revised by the following amount:
a. Title CI funds increase by $25,000.00
This amendment shall be effective on November 1,2009. All provisions in the contract and
any Attachments thereto in conflict with this amendment shall be hereby changed to conform
with this amendment.
All provisions not in conflict with this amendment are still in effect and are to be performed at
the level specified in the contract.
This amendment and all its attachments are herby made a part of the agreement.
IN WITNESS THEREOF, the parties hereto have caused this 2 page contract to be executed
by their undersigned officials as duly authorized.
