OBJECTIVE: That the Board of County Commissioners approves and authorizes the Chairman to sign an amendment between the Collier County Board of County Commissioners and the Area Agency on Aging for Southwest Florida, Inc. to reflect additional grant funding in the amount of $699.34 for the Nutritional Services Incentive PrObTfam (NSIP). CONSIDERATIONS: In January 2008, the Housing and Human Services Department, Services for Seniors program, received approval from the Board to begin administering the Senior Nutrition Program. The program in Collier County is partially funded through the Federal Nutritional Services Incentive Program (NSIP) and currently consists of a congregate meal site in East Naples, Golden Gate and Immokalee. Home delivered meals are also provided to frail, home bound, elderly through this grant program. The original NSIP agreement for FY09 in the amount of $43,255.32 was approved by the Board on October 28, 2008 item 16D23. In order to receive the additional funding for FY09, an amendment in the amount of $699.34 must be executed by the Board of County Commissioners. FISCAL IMPACT: The Senior Nutrition Program budget will increase by $699.34. Matching funds for this program are not required. LEGAL CONSIDERATIONS: This is a forn1 amendment provided by the Area Agency on Aging for Southwest Florida, Inc. This amendment has been reviewed by the County Attorney's Office and requires majority vote only. This contract is subject to all provisions contained in the CONTRACT executed between the Agency and the vendor, Contract No. NSIP 203.09, and its successor, incorporated herein by reference. The purpose of this amendment is to: (1) amend Paragraph 4 of the contract to increase the contract amount by $699.34 and increase the level of service accordingly; (2) amend Paragraph 3.1 of Attachment I and (3) revise and replace Attachment III, Exhibit-I. This amendment shall be effective on Jooe 1,2009. All provisions in the contract and any Attachments thereto in conflict with this amendment shall be hereby changed to conform with this amendment. All provisions not in conflict with this amendment are still in effect and are to be performed at the level specified in the contract. This amendment and all its attachments are herby made a part of the agreement.
