EXECUTIVE SUMMARY
Recommendation to approve and authorize the Chairman to sign an amendment between
Collier County Board of County Commissioners and the Area Agency on Aging for
Southwest Florida, Inc., which reflects additional grant funding in the amount of $23,265
for the Community Care for the Elderly (CCE) Program for 2009.
OBJECTIVE: To amend the 2009 Community Care for the Elderly grant agreement between
the Board of County Commissioners and the Area Agency on Aging. This amendment provides
an increase of $23,265 in additional funding.
CONSIDERATIONS: Collier County Services for Seniors program has been providing support
services to Collier County's frail elderly for over 30 years through the Community Care for the
Elderly program. The services coordinated by staff in the Services for Seniors program allows
area seniors to remain in their homes and age with dignity by providing services such as
homemaker, personal care, respite, emergency alert response, and medical supplies. Collier
County Services for Seniors has received an additional allocation in the FY09 Community Care
for the Elderly program in the amount of $23,265. Therefore, an amendment to the original
agreement must be approved to accept the additional funding. The original agreement was
approved by the BCC on 10/14/08 item 16D25.
/- The total grant award now amounts to $672,993 and is for the period .Tuly 1, 2008 through .Tune
30,2009. The amendment for the additional funding must be signed by the Chairman.
GROWTH MANAGEMENT: None.
FISCAL IMPACT: The funding in the amount was increased by $23,265. No additional
matching funds are required.
LEGAL CONSIDERATIONS: This item is not quasi judicial and as such ex parte disclosure
is not required. This item requires majority vote only. This item is legally sufficient for Board
action. -CMG
RECOMMENDATIONS: That the Board of County Commissioners approve and authorize the
Chairman to sign the grant amendment with the Area Agency on Aging for Southwest Florida,
Inc..

STANDARD CONTRACT
AREA AGENCY ON AGING
Collier County Services for Seniors
This AMENDMENT, entered into by the Area Agency on Aging for Southwest Floricfu, Inc.,
hereinafter referred to as the "agency", and Collier County Services for Seniors. hereinafter
referred to as the "recipient", amends agreement #CCE 203.08.
The purpose of this amendment is to;
1. adjust the contract amount by the following, effective April 10, 2009:
A: increase the lead agency funds by $3,490.00
B: increase spending authority funds by $19,775.00, and
2. revise the Budget Summary, Attachment III.
All provisions in the agreement and any attachments thereto in conflict with this amendment
shall be and are hereby changed to conform with this amendment.
All provisions not in conflict with this amendment are still in effect and are to be performed at
the level specified in the agreement.
This amendment and all of its attachments are hereby made a part of this agreement.
IN WITNESS WHEREOF, the parties hereto have caused this ~page amendment to be executed
