Executive SUmmary
Recommendation that the Board of County Commissioners approves and authorizes its
Chairman to si"" a modification to Disaster Recoverv Grant Initiative A!!reement #07DB-
'" . ~
3V-09-21-01-Z01 between the Florida Department of Community Affairs (DCA) and
Collier County. This modification wiII add new activities to utilize grant funds previously
awarded St. Matthews House.
OBJECTIVE: To bave tbc Board of County Commissioners approve and autborize its
Chairman to sign a modification to Di:;aster Recovery Grant Initiative (DRI) agreemcnt between
the Florida Department of Community A mlirs and Collier County.
CONSIDERATION: On May 23, 2007 tbe Board of COll11ty Commissioncrs approved the
Disaster Recovery Initiative Grant Agreement #07DB-3V-OC)-21-01-Z01 with the Florida
Department of Community Affairs (DCA). The Agreement establishes several hurricane
hardening projects for utilizing the grant funds. St. Matthew's House's Subrecipient Agreements
for projects with this grant wcre approved by tbe Board of County Commissioners on November
13,2007.
At this time, modification is needed 10 grant agreement #07DB-3 V -OC)-2l-0 I-ZO I to rellect
necessary changes for one of the projects.
For the St. Matthew's House hurricane bardening project, a portion of the awarded funds needs
to be reallocated to a new activity. Originally the project inciuded the purchase and installation
of hurricane resistant \vindo\vs, sliding doors and a generator for their 11131n facility located at
2001 Airport Road South in Naples. The bids on this project have come in well under budget,
and at this point the project is more than 90% complete. The grant agreement awarded St.
Matthew's House $293,934 for the original project. St. Matthew's House has asked for
hurricane hardening to continue With installation of up to I 5 hurricane resistancc exterior doors at
their main facility and a generator at their auxiliary emergency shelter located at 2691 Airport
Road South in Naples. Funds used at the auxiliary emergency shelter will increase the number
of beneficiaries with this grant lI'om 450 to 566. To Llcilitate these extra activities this
modification to the grant agreement with DCA will also provide a six month extension,
extending the end date of the grant li'om June 19,2009 to December 19,2009.
The trade contract between St. Matthews House and Redic construction will be modified to
include the additional generator and up to IS exterior cloors. The modification to the trade
agreement will not exceed $7~.Sb6. 72 which is the amount of tbe rcmaining un-contracted grant
funds for this project.

is not required. This item requires majority vote only. This itcm is legally sufficient for Board

MODIFICATION NUMBER-L TO GRANT AGREEMENT BETWEEN
THE DEPARTMENT OF COMMUNITY AFFAIRS AND
COLLIER COUNTY
This Modification is made and entered into by and between the State of Florida,
Department of Community Affairs, (the Department"), and Collier Countv
(the Recipient"), to modify DCA Contract Number 07DB-3V-09-21-01-Z01, award
dated May 23. 2007 ,("the Agreement").
WHEREAS, the Department and the Recipient entered into the Agreement,
pursuant to which the Department provided a grant of $ 2.339.882 to Recipient under
the Small Cities Community Development Block Grant ("CDBG") Program as set forth
in the Agreement;
WHEREAS, the Department and the Recipient desire to modifY the Agreement;
NOW, THEREFORE, in consideration of the mutual promises of the parties
contained herein, the parties agree as follows:
o Reinstate Agreement
I. The Agreement is hereby reinstated as though it had not expired.
X Extend Agreement
2. Paragraph 3, Period of Agreement is hereby revised to reflect an ending date
of_December 19. 2009
X Revise Activity Work Plan
3. The Attachment B, Activity Work Plan section of the Agreement is hereby
deleted and is replaced by the revised Attachment B, Activity Work Plan
section, which is attached hereto and incorporated herein by reference.
o Revise Program Budget and Scope of Work
4. The Attachment A, Program Budget and Scope of Work section of the
Agreement is hereby deleted and is replaced by the revised Attachment A, the
Program Budget and Scope of Work, which is attached hereto and
incorporated herein by reference.

5. The Attachment G, Special Conditions section, is hereby modified to delete
all references to << ," as the Participating
Party, and replace them with << " as the
Participating Party with the understanding that the Recipient and the new
Participating Party will enter into a Participating Party Agreement containing
provisions and caveats that meet or exceed the conditions agreed to in the
Participating Party Agreement between the Recipient and the original
Participating Party.
o Inclusion of an Unmet Need as Addressed in the Original Application
6. The Attachment A, Program Budget and Scope of Work section of the
Agreement is hereby deleted and is replaced by the revised Attachment A, the
Program Budget and Scope of Work, which is attached hereto and
incorporated herein by reference.
7. The Attachment B, Activity Work Plan section of the Agreement is hereby
deleted and is replaced by the revised Attachment B, Activity Work Plan
section, which is attached hereto and incorporated herein by rcference.
X Change in Number of Accomplishments and/or Beneficiaries
8. The Attachment A, Program Budget and Scope of Work section of the
Agreement is hereby deleted and is replaced by the revised Attachment A, the
Program Budget and Scope of Work, which is attached hereto and
incorporated herein by reference.
All provisions of the Agreement and any attachments thereto in conflict with this
Modification shall be and are hereby changed to conform to this Modification, effective
as of the date of the execution of this Modification by both parties.
All provisions not in conflict with this Modification remain in full force and
effect, and are to be performed at the level specified in the Agreement.
