EXECUTIVE SUMMARY
Recommendation to approve a Certificate of Public Convenience and Necessity for a Class 2 non-
emergency inter-facility ambulance transports to Just Like Family Concierge Medical Transport
Services, LLC (D.B.A. Concierge Medical Transport) to allow post-hospital inter-facility
ambulance transfer services.
OBJECTIVE: To proceed in the best interest of the public health, safety and welfare by granting
a Certificate of Public Convenience and Necessity (COPCN), Class 2 Advanced Life Support (ALS)
inter-facility ambulance transport certificate(s) to Just Like Family Concierge Medical Transport, LLC
(D.B.A. Concierge Medical Transport). These certificates are subject to certain conditions noted below.
CONSIDERATIONS: Just Like Family Concierge Medical Transport, LLC (D.B.A. Concierge Medical
Transport) was originally granted a Certificate of Public Convenience and Necessity on March 8, 2018 by
the Board. The certificate requires annual renewal and the current COPCN expires on March 7, 2019. A
renewal application submitted by Concierge Medical Transport is being reviewed as a new application
because of a change in ownership.
Section 50-57 of the Collier Code and Ordinances sets for the requirements for granting a certificate as
follows:
Sec. 50-57. - Requirement for board approval in granting certificate.
The Board of County Commissioners shall not grant a certificate unless it shall find, after public
hearing and based on competent evidence that each of the following standards has been satisfied:
(1) That there is a public necessity for the service. In making such determination, the Board of
County Commissioners shall consider, as a minimum, the following factors:
a. The extent to which the proposed service is needed to improve the overall emergency
medical services (EMS) capabilities of the County.
b. The effect of the proposed service on existing services with respect to quality of service
and cost of service.
c. The effect of the proposed service on the overall cost of EMS service in the county.
d. The effect of the proposed service on existing hospitals and other health care facilities.
e. The effect of the proposed service on personnel of existing services and the availability
of sufficient qualified personnel in the local area to adequately staff all existing
services.
(2) That the applicant has sufficient knowledge and experience to properly operate the
proposed service.
(3) That, if applicable, there is an adequate revenue base for the proposed service.
(4) That the proposed service will have sufficient personnel and equipment to adequately cover
the proposed service area.
These requirements have been met and the administrator recommends renewal. The Emergency
Medical Authority reviewed the application on February 13, 2019 and unanimously recommended
that the Certificate be approved.

FISCAL IMPACT: There is no fiscal impact to the Board for the granting of this certificate. The
Board is requested to accept and recognize as revenue in Fund (001) the $250.00 application fee with the
corresponding application.
GROWTH MANAGEMENT IMPACT: There is no Growth Management Impact resulting from
this action.
LEGAL CONSIDERATIONS: Section 50-57 of the Collier Code and Ordinances as fully set forth
above, establishes the criterion for Board consideration of the application.
Additionally, the Board has the option to appoint a hearing office as provided by Section 50 -58 of the
Collier County Code of Laws and Ordinances which states:
Sec. 50-58. - Appointment of hearing officer.
In making the determinations provided for in section 50-57 above, the board may, in its sole
discretion, appoint a hearing officer to hold a public hearing and to make factual findings and
conclusions as a result of the hearing. Should a Hearing Officer be appointed, said hearing officer
shall render a written report to the board within 30 days of the hearing, which report shall contain the
officer's findings and conclusions of fact, and a recommended order. The findings and conclusions of
fact shall be binding upon the board, but the recommended order shall be advisory only.
Accordingly, this item is approved for form and legality and requires a majority vote for Board action.

RECOMMENDATION: That the Board of County Commissioners:
1) Make findings that:
a. There is a public necessity for the service, and
b. The applicant has sufficient knowledge and experience to properly operate the proposed
service, and
c. There is an adequate revenue base for the proposed service, and
d. The the proposed service will have sufficient personnel and equipment to adequately
cover the proposed service area; and
2) Approve and authorize the Chairman to execute a Class 2 Certificate of Public Convenience and
Necessity to Just Like Family Concierge Medical Transport , LLC (D.B.A. Concierge Medical
Transport) for one year.

An applicant for a certificate shall obtain form from the department to be completed and
returned to the division administrator. Each application shall contain:
1. The name age and address of the owner of the ambulance or ALS provider,
or if the owner is a corporation, then of the directors of the corporation and
all of the stockholders holding more than 25 percent of the outstanding
shares. For governmental units, this information shall be supplied for
members of the governing body.

Concierge Medical Transport currently has five (5) licensed ambulances in its SWF division.
Currently there are two (2) ambulances permitted by the Florida Department of Health,
Bureau of EMS as Advanced Life Support vehicles.
ln addition, there are three (3) ambulances permitted by the Florida Department of Health,
Bureau of EMS as Advanced Life Support and Basic Life Support vehicles.
Using Zoll Data Rescue.Net Dispatch software, we are able to track the current and
anticipated demands for service. Using Fleetlocate by Spireon we are able to track (in real
time) our vehicles exact location and provide more accurate arrival times to our customers,
thus improving the patient flow at the local hospitals.
Attached you will find the list of all referenced ambulances and the current ALS/BLS permit
numbers (appendix "i").

5. The training and experience of the applicant.
The management team at Concierge Medical Transport collectively has over 300 years of
experience in the EMS industry with varied experience in Field training, education, management,
air medical and critical care transport. With over a century in the ambulance industry, we can
provide the expert leadership needed to run a respectable, high quality service. Our goal is to be
a leader in the patient transport industry by being a role model organization in everything we do
from how we care for our employees to the investments we make in our technology and vehicles.
Our Quality Assurance Department is staffed by l0 full+ime Paramedics All QA staff have a
minimum of l0 years with a Paramedic certification. Our QA staffreviews patient care reports
for quality assurance and adherence to our medical protocols and standards of care. Concierge
Medical Transport is committed to maintaining compliance with all federal, state and local laws
and regulations. We are compliant with Florida Statute 401 and Florida Administrative Code 64f
in accordance with standards set forth by the Bureau of EMS and Department of Health. Our
state license is valid through May 15,2020 A copy of our state license is attached hereto as
appendix "B."
Concierge Medical Transport accepts all insurance providers. All billing and collections are done
in house with our team of close to 60, to ensure patient confidentiality and a payment plan for all
patients with financially difficulty. We offer a centralized billing office staffed with billers that
have a minimum of 10 years' experience and receive extensive ongoing training. CMT is proud
of their high rate of customer satisfaction with minimal billing errors or concerns.
All CMT field employees maintain their credentials in a proactive manner as well as pursue
continuing education tracks so they achieve individual career and certification objectives. During
orientation and throughout the year, all employees are required to take a course or refresher
course in infection control, airborne pathogens, blood-borne pathogens, HIPPA, injury
prevention, and emergency vehicle operations. EMTs and Paramedics are responsible for
maintaining their own credentials and certifications. Concierge Medical Transport sponsors
continuing education support in order for employees to maintain their certifications.
The required l6-hour Emergency Vehicles Operators Course is handled by our in house EVOC
Instructor. Who uses a combination of classroom and practical driving to train staffon the proper
driving techniques to safely drive an emergency vehicle in the state of Florida.

Dr. Hobaica is a graduate of Bridgewater State University I Business Administration]. prior to
moving from Massachusetts to Naples in 1999, he served with distinction at the University of
Massachusetts Medical Center. A board-certified physician in internal medicine, Dr. Hobaica
served in the Emergency Department at Naples Community Hospital INCHI prior to entering
private practice.
As you may know, Dr. Hobaica serves as Medical Director for Arthrex where from its Naples
global headquarters, he leads a team providing primary care services offered to Arthrex's nearly
3000 employees office here and in regional offices.
But, perhaps, you may not be fully aware that Dr. Hobaica also has provided health care
services to many of our public protection officers and has always been available,24fi to assist
them in immediate personal care and emergency matters.
Paul Hobaica has also greatly assisted me in founding Step Smart Naples which now
incorporated as Step Smart Collier has had a significant impact in educating both our general
public and our seniors in fall accident prevention. While I served on the Advisory Board of our
five-county Regional Trauma Center, I learned that our Collier County Emergency Medical
Service [EMS] responded to an alarming number of accidents, some traumatic, which resulted
in hospital and in severe cases Trauma Center admissions
Dr. Hobaica assisted me in hosting 30 local leaders for a day-long workshop sponsored byArthrex to craft an educational/information program to provide guidance and advice to thegeneral public on how to reduce falls, especially in our residences. Several organizations,
AARP-Florida, Sunshine ACE Hardware, the Napes Daily News, Waterman Bioadcast, amongothers, then supported the production of a video and provision of instructional materials thatwere used in public service announcements, newspaper guest commentaries and over 50

forums throughout our county. Dr. Hobaica and his colleagues provided me with episodic private
advice as we advanced this effort into today what is known as the remarkably successful Step
Smart Collier that is credited, through its public information campaigns and forums to having a
reduction impact on injuries from falls in Collier County.
ln my opinion, Paul Hobaica is practically dedicated to helping to do positively what you can,
where you are and with what you have. ln medical circles, he is regarded as being most credible
and possessing uncompromising integrity. Whenever over the years I have quiefly asked for his
advice, he has always stressed engaging others thru education to improve services and solve
problems.
Many know stories of Dr. Hobaica privately and enthusiastically, pro bono, stopping to assist
folks in trouble. He often demonstrated an indefatigable eagerness to try to positively advance
our community... without seeking any public recognition or personal aggrandizement.

the JUST LIKE FAMILY CONCIERGE MEDICAL TRANSPORT SERVICES, LLC, (hereinafter "Just
Like Family") DBA Concierge Medical Transport wishes to provide post-hospital inter-facility ftansports

Per our phone discussion, you currently do not have or need tail coverage. Tail coverage is needed when a claims made
policy ends if there is no renewal. You have had continuous coverage, therefore tail coverage is not needed. Once the
sale with Brewster Ambulance is complete, there may be a need for this coverage. lf this is the case, we will put it in
