State of Florida, in accordance with a signed agreement dated November 13, 2012 between Youth Haven,
Inc. and Collier County.
This First Amendment amends the Declaration of Restrictive Covenants dated July 31, 2014 and
recorded September 10, 2014 in Official Records Book 5075, Page 2088, of the Public Records of Collier
County,Florida(the"Declaration of Restrictive Covenants").
WITNESSETH:
WHEREAS,in accordance with the terms of the agreement between Youth Haven,Inc.and Collier
County dated November 13, 2012 and 24 CFR 570.503, Youth Haven, Inc. executed the Declaration of
Restrictive Covenants; and
WHEREAS, the parties of the November 13, 2012 agreement desire to amend the Declaration of
Restrictive Covenants to change the legal description of the property affected thereby.
NOW, THEREFORE, the Declaration of Restrictive Covenants is hereby amended by releasing
therefrom the property legally described in Exhibit B attached hereto and hereby made a part hereof. The
property now affected and encumbered by the Declaration of Restrictive Covenants is legally described in
