SECOND AMENDMENT
THIS SECOND AMENDMENT is effective as of June 24, 2010, by and between COLLIER
COUNTY, A POLITICAL SUBDIVISION OF THE STATE OF FLORIA, whose mailing
address is 3301 Tamiemi Trail East, Naples. FL 34112 (hereinafter referred to as "County"). and
DAVID LAWRENCE MENTAL HEALTH CEN'T'ER, INC., a Florida non -profit
corporation, d/b /a David Lawrence Center, whose mailing address is 6075 ButhLy Lane.
Naplcs, FL 34116 (hereinafter referred to as "DLC ").
County and DLC entered into that certain Real Estate Conveyance Agreement dated October 27,
2009 ( "the Agreement') and Amendment to Agreement effective February 24, 2010. Pursuant m
the provisions of the Agreement, County and DLC wish to amend the Agreement with the
following provisions:
1. DLC wishes to extend the due diligence period an additional 120 days, bringing the due
diligence period to October 22, 2010.
IN WITNESS WHEREOF, the County and DLC have hereto executed this Second Amendment
on the dates set Forth below.
