EXECUTIVE SUMMARY
Recommendation to provide after-the-fact approval for the submittal of a Library Services and
Technology Act (LSTA) Supporting Public Library Innovation Grant application to the Florida
Department of State, Division of Library and Information Services for funding in the estimated
amount of $4,000.
OBJECTIVE: To obtain LSTA grant funds in the estimated amount of $4,000.
CONSIDERATIONS: The number of young adults entering the work force and college without the
basic life skills to succeed continues to increase. Public schools no longer teach basic skills such as
money management, cooking, sewing, and etiquette once the standard curriculum of a home economics
class. Public libraries are now creating makerspaces and developing programs to address this deficit in
educating our youth.
Grant funds will be used to purchase sewing machines, crafting supplies, and cooking supplies to expand
the Library’s programming offerings. A mobile, low-tech makerspace will be created for a newly
developed young adult program “Insfor Teens.” The program is designed to introduce basic life skills
through a series of classes covering simple sewing and crafting, cooking, etiquette, budgeting, and home
care.
Demand for makerspaces such as this was a key recommendation from respondents in the Library’s 2015
Community Assessment.
The Florida Department of State, Division of Library and Information Services is the state library
administrative agency officially designated to receive and administer federal grant funds from the
Institute of Museum and Library Services. The LSTA is the major federal grant program for libraries.
The LSTA Program, administered by the Institute of Museum and Library Services, has purposes which
focus on information access through technology and information empowerment through special services,
and is authorized under 20 United States Code 9101 et seq, as amended. As a Florida library, Collier
County Public Library is eligible to apply for LSTA funds.
Notification and preparation of the grant occurred in September leaving insufficient time for Board action
to meet the grant submission deadline of October 4, 2018. Collier County CMA #5330 authorizes the
County Manager to approve the submittal of grant applications with subsequent Board of County
Commissioners (Board) action at the next available Board Meeting to ratify the approval as “after -the-
fact.” The Board will have the opportunity to accept or reject the funds if the grant is approved.
FISCAL IMPACT: Maximum federal grant funding is $4,000. Upon notification of the grant award, the
Board will be asked to accept the grant and approve the Budget Amendment to recognize the revenue and
appropriate the expenditures. There is no impact to the General Fund with this grant.
LEGAL CONSIDERATIONS: The Board will have the opportunity to accept or reject the funds if the
grant is approved. Accordingly, this Office has no issue with respect to the legality of this request, which
is appropriate for Board action and requires a majority vote for Board action. -JAB
GROWTH MANAGEMENT IMPACT: There is no Growth Management impact associated with this
request.
RECOMMENDATION: To provide after-the-fact approval for the submittal of the LSTA Supporting
Public Library Innovation Grant application for funding in the estimated amount of $4,000.

The pilot program will have the items being used by teens at Collier County
Headquarters Library,2385 Orange Blossom Dr., Naples, FL.
After the initial pilot program, the items will be made part of a mobile lab for use at
all ten Collier County Libraries.
For the proposed series program "Life Skills Bootcamp for Teens", Collier County Public
Library will purchase:
1) Four basic sewing machines
2) One computerized sewing machine adapted for those with physical limitations;
3) Four Cricut Maker lvlachines & accessories/supplies;
4) sewing accessories: rotary cutters, rotary blades, cutting mats, rulers, straight pins,
shears, tape measures, seam rippers, needles, thread, fabric, and bobbins;
5)Three crock-pots.
These items will be used in the "Life Skills Bootcamp for Teens" program that will introduce basic
life skills in an informative yet fun fashion over the course of one year in a series of classes that
focuses on easy and inexpensive cooking, simple sewing and crafting, and a series of "life hacks"
for navigating adulthood such as etiquette, entertaining, budgeting, and home care.
lronically, with the advent and ubiquity of technology, life skills previously taught in "old school"
home economics classes are now the "new" innovations, and this grant will help support our teens
in learning these necessary skills.

INSTRUCTIONS FOR CERTIFICATION
1. By signing and submitting this proposal, the prospective lower tier participant is providing the certification set
out below.
2. The certification in this clause is a material representation of fact upon which reliance was placed when this
transaction was entered into. If it is later determined that the prospective lower tier participant knowingly
rendered an erroneous certification, in addition to other remedies available to the federal government, the
department or agency with which this transaction originated may pursue available remedies, including
suspension and/or debarment.
3. The prospective lower tier participant shall provide immediate written notice to the person to which this
proposal is submitted if at any time the prospective lower tier participant learns that its certification was
erroneous when submitted or has become erroneous by reason of changed circumstances.
4. The terms “covered transaction,” “debarred,” “suspended,” “ineligible,” “lower tier covered transaction,”
“participant,” “person,” “primary covered transaction,” “principal,” “proposal,” and “voluntarily excluded,” as
used in this clause, have the meanings set out in the Definitions and Coverage sections of rules implementing
Executive Order 12549. You may contact the person to which this proposal is submitted for assistance in
obtaining a copy of those regulations.
5. The prospective lower tier participant agrees by submitting this proposal that, should the proposed covered
transaction be entered into, it shall not knowingly enter into any lower tier covered transaction with a person
who is debarred, suspended, declared ineligible, or voluntarily excluded from participation in this covered
transaction, unless authorized by the department or agency with which this transaction originated.
6. The prospective lower tier participant further agrees by submitting this proposal that it will include the clause
titled “Certification Regarding Debarment, Suspension, Ineligibility, and Voluntary Exclusion-Lower Tier
Covered Transactions,” without modification, in all lower tier covered transactions and in all solicitations for
lower tier covered transactions.
7. A participant in a covered transaction may rely upon a certification of a prospective participant in a lower tier
covered transaction that it is not debarred, suspended, ineligible, or voluntarily excluded from the covered
transaction, unless it knows that the certification is erroneous. A participant may decide the method and
frequency by which it determines the eligibility of its principals. Each participant may, but is not required to,
check the Nonprocurement List (Telephone Number).
8. Nothing contained in the foregoing shall be construed to require establishment of a system of records in order
to render in good faith the certification required by this clause. The knowledge and information of a
participant is not required to exceed that which is normally possessed by a prudent person in the ordinary
course of business dealings.
9. Except for transactions authorized under paragraph 5 of these instructions, if a participant in a covered
transaction knowingly enters into a lower tier covered transaction with a person who is suspended, debarred,
ineligible, or voluntary excluded from participation in this transaction, in addition to other remedies available
to the federal government, the department or agency with which this transaction originated may pursue
available remedies, including suspension and/or debarment.

Federal Funding Accountability and Transparency Act (FFATA) Reporting
Subgrantees must complete the Federal Funding Accountability and Transparency Act
Reporting form. The FFATA Act requires that information on federal awards and
executive compensation be made available to the public through a single searchable
website, www.USASpending.gov.
Applicability and What to Report
A. If, in the previous fiscal year, the subgrantee’s gross income from all sources was
under $300,000, it is exempt from the requirements to report:
1. Subawards, and
2. The total compensation of the five most highly compensated executives of the
subgrantee.
B. If the subgrantee is not exempt as provided in paragraph A., the subgrantee shall
report the names and total compensation of each of its five most highly
compensated executives, if -
1. In the subgrantee’s preceding fiscal year, the subgrantee received:
a) 80 percent or more of its annual gross revenues from Federal procurement
contracts (and subcontracts) and Federal financial assistance subject to the
Transparency Act, as defined at 2 CFR 170.320 (and subawards); and
b) $25,000,000 or more in annual gross revenues from Federal procurement
contracts (and subcontracts), and Federal financial assistance subject to the
Transparency Act (and subgrantees); and
2. The public does not have access to information about the compensation of the
executives through periodic reports filed under section 13(a) or 15(d) of the
Securities Exchange Act of 1934 (15 U.S.C. 78m(a), 78o(d)) or section 6104 of
the Internal Revenue Code of 1986. (To determine if the public has access to the

Definitions
Total compensation means the cash and noncash dollar value earned by the
executive during the subgrantee's preceding fiscal year and includes the following (for
more information see 17 CFR 229.402(c)(2)):
i. Salary and bonus.
ii. Awards of stock, stock options, and stock appreciation rights. Use the dollar
amount recognized for financial statement reporting purposes with respect to
the fiscal year in accordance with the Statement of Financial Accounting
Standards No. 123 (Revised 2004) (FAS 123R), Shared Based Payments.
iii. Earnings for services under non-equity incentive plans. This does not include
group life, health, hospitalization or medical reimbursement plans that do not
discriminate in favor of executives, and are available generally to all salaried
employees.
iv. Change in pension value. This is the change in present value of defined
benefit and actuarial pension plans.
v. Above-market earnings on deferred compensation which is not tax-qualified.
vi. Other compensation, if the aggregate value of all such other compensation
(e.g., severance, termination payments, value of life insurance paid on behalf
of the employee, perquisites or property) for the executive exceeds $10,000.
Executive means officers, managing partners, or any other employees in management
positions.
Data Universal Numbering System (DUNS) number means the nine-digit number
established and assigned by Dun and Bradstreet, Inc. (D&B) to uniquely identify
business entities. A DUNS number may be obtained from D&B by telephone
(866.705.5711) or the Internet (http://fedgov.dnb.com/webform).
