AMEMDMENT NO. 3 TO THE LETTER OF AGREEMENT
FOR BLUEPRINT FOR WELLNESS SERVICES
BETWEEN
COLLIER COUNTY GOVERNMENT
AND QUEST DIAGNOSTICS INCORPORATED
This AMENDMENT NO.3 to the Letter of Agreement for Blueprint for Wellness Services ( "Amendment No.2 "),
effective as of the 27th day of March 2012 ( "Effective Date") is entered into by and between Quest Diagnostics
Incorporated ( "Quest Diagnostics ") and Collier County Government ( "Client") and amends the existing Letter of
Agreement for Blueprint for Wellness Services.
WITNESSETH:
WHEREAS, Quest Diagnostics and Client entered into a certain Letter of Agreement for Blueprint for Wellness
Services on the 12th day of July, 2010, as amended by a First Amendment effective on the 21St day of March, 2011
(collectively the "Agreement ") and by a Second Amendment effective on the 1" day of March, 2012, all of which
incorporated herein by reference; and
WHEREAS, Quest Diagnostics and Client wish to extend the term of the Agreement.
NOW, THEREFORE, in consideration of the mutual promises hereinafter set forth, the receipt and sufficiency of
which are hereby acknowledged, the parties hereby agree as follows:
1. The term of the Agreement shall be extended until September 30, 2012 or until the new contract is
awarded, whichever comes sooner.
All other terms of the Agreement, including the Attachment(s) thereto, not specifically modified by this
Amendment No. 3 remain in full force and effect.
