SECOND AMENDMENT OF AGREEMENT BETWEEN
Collier County Government AND
QUEST DIAGNOSTICS INCORPORATED
THIS SECOND AMENDMENT is effective as of the 17th day of October, 2011, by and between Quest
Diagnostics Incorporated, with a principal place of business located at 10101 Renner Blvd., Lenexa, KS
66219 ( "Quest Diagnostics "), and Collier County Government, 3301 East Tamiami Trail, Bldg. D, Naples,
FL 34112.
WHEREAS, the parties have previously entered into. a 2011 Letter of Agreement & Event Scheduling
Memo. for Blueprint for Wellness Services on the 12th day of July, 2010, and a First Amendment dated
March 21, 2011, for the performance of Blueprint for Wellness Services, (collectively, the "Agreement ");
and
WHEREAS, the parties, by this Second Amendment, desire to amend the Agreement by adding 2
additional testing panels.
NOW, THEREFORE, for and in consideration of the covenants and agreements made herein and of the
mutual benefits occasioned by the terms of this Second Amendment, the presence of which are mutually
acknowledged and confessed, the parties each agree as follows:
The following two additional testing panels shall be added to the Agreement:
1. Lipid Panel - $34.50 per participant
2. Hemoglobin Al c Test — $24.00 per participant
