THIS AMENDMENT is entered into between the Area Agency on Aging for Southwest Florida, Inc. dba Senior Choices
of Southwest Florida ("Agency") and Collier County Board of County Commissioners, ("Recipient"), amends agreement
OAA 203.13.
The purpose of this amendment is to increase the OA3B In -Home services amount by $9,068 and Move $31,200 from
Transportation to In -Home services and Move $4,500 from Transportation to Case Management/Case Aid. This
amendment changes the total OA3B In -Home Services from $340,500 to $380,768, Case Management/Case Aid to
$26,000, and Transportation to $29,300 which changes total contract funding from $732,962.00 to $700,862.00 affecting
Year Two, 2013-2014; and revise ATTACHMENT III EXHIBIT — I and ATTACHMENT VII, Budget Summary.
This amendment shall be effective on October 3, 2013. All provisions in the agreement and any attachments thereto in
conflict with this amendment shall be and are hereby changed to conform with this amendment.
All provisions not in conflict with this amendment are still in effect and are to be performed at the level specified in the
agreement.
This amendment and all of its attachments are hereby made a part of this agreement.
