5. The Chairman's signature line date has been entered as the date of BCC approval of the SRT
_ document or the final negotiated contract date whichever is applicable.
6. "Sign here"tabs are placed on the appropriate pages indicating where the Chairman's SRT
_ signature and initials are required.
7. In most cases(some contracts are an exception),the original document and this routing slip SRT
should be provided to the County Attorney Office at the time the item is input into SIRE.
Some documents are time sensitive and require forwarding to Tallahassee within a certain
time frame or the BCC's actions are nullified. Be aware of your deadlines!
8. The document was approved by the BCC on 11/12/13 and all changes made during
the meeting have been incorporated in the attached document. The County
Attorney's Office has reviewed the changes,if applicable.

Dear Ms. Arnold:
Your agency has complied with all of the program requirements necessary to qualify as a
recipient of an annual allocation of State Transit Block Grant Program funding. As you may
recall. a multi-year Joint Participation Agreement (IPA) for the State Transit Block Grant
Program was executed on October 16. 2012: hence. annual JPAs are no longer required.
I lowever, a Multi-Year WA Notification of Funding form is needed and is attached.
The firm has a space for a required signature from the authorized agency representative as per
the Resolution. Once signed. the limn should be returned to me for further processing. Should

NOTIFICATION OF FUNDING
In accordance with the terms of the Joint Participation Agreement between
Collier County Board of County Commissioners,
3299 East Tamiami Trial, Naples, Florida 34112 and the Department,
State Transit Block Grant Program identified by contract number A0016 and
project number 410139-1-84-01; the Department has encumbered $907,341 using
project number 410139-1-84-01.
Upon signature by your agency and the Department, eligible project costs may be
incurred. Costs incurred prior to full execution of this form will not be eligible
for reimbursement.
