Backup Documents 05/14/2013 Item # 4AP R 0 C L A M A TION WHEREAS, in 2003, the Collier County Medical Society (CCMS) recognized a need for specialty medical care for low income uninsured adults in the county; and, WHEREAS, the CCMS had several hundred members who were specialists and decided to create the Physician Led Access Network or PLAN to meet this need, and, WHEREAS, the staff at PLAN take patient referrals from primary care safety net providers, assure patients are income qualified at less than 200% of the Federal Poverty Level and make appointments with the next available appropriate specialist member of PLAN; and, WHEREAS, this organized method of rotating specialty medical care helps to assure that no one volunteer specialist is overly burdened with 'uninsured patients; and, WHEREAS, PLAN specialists include dermatologists, cardiologists, urologists, radiologists, oncologists, surgeons and others; and, WHEREAS, volunteer CCMS physicians through PLAN have treated over 3,000 county residents and provided $18 million worth of free specialty medical care to the community over the past ten years. NOW THEREFORE, be it proclaimed by the Board of County Commissioners of Collier County, Florida, that May 2013 be designated as COLLIER COUNTY PHYSICIAN LED ACCESS NETWORK MONTH DONE AND ORDERED THIS 14th day of May, 2013.
