1. Original document has been signed/initialed for legal sufficiency. (All documents to be
signed by the Chairman, with the exception of most letters, must be reviewed and signed
by the Office of the County Attorney. This includes signature pages from ordinances,
resolutions,etc. signed by the County Attorney's Office and signature pages from
contracts,agreements, etc. that have been fully executed by all parties except the BCC
Chairman and Clerk to the Board and possibly State Officials.)
2. All handwritten strike-through and revisions have been initialed by the County Attorney's N/A
Office and all other parties except the BCC Chairman and the Clerk to the Board
3. The Chairman's signature line date has been entered as the date of BCC approval of the N/A
_ document or the final negotiated contract date whichever is applicable.
4. "Sign here"tabs are placed on the appropriate pages indicating where the Chairman's
signature and initials are required.
5. In most cases(some contracts are an exception),the original document and this routing slip
should be provided to BCC Office Supervisor in the BCC office within 24 hours of BCC

a certain time frame or the BCC's actions are nullified. Be aware of your deadlines!

SOUTH FLORIDA WATER MANAGEMENT DISTRICT
AND
BOARD OF COUNTY COMMISSIONERS, COLLIER COUNTY, FLORIDA
2013 This AMENDMENT NO. 02, is entered into on MAR 2 8 to
that AGREEMENT dated March 12, 2012, and as amended on November 28, 2012, between
"the Parties," the South Florida Water Management District (DISTRICT), and Board of County
Commissioners, Collier County, Florida (COUNTY).
WITNESSETH THAT:
WHEREAS, the AGREEMENT may be amended with the prior written approval of the
Parties; and
WHEREAS, the Governing Board of the DISTRICT, at its November 15, 2012 meeting,
approved entering into this AMENDMENT NO. 02 with the COUNTY; and
WHEREAS, the Parties wish to amend the AGREEMENT in order to revise the
Payment and Deliverable Schedule of the AGREEMENT;
NOW THEREFORE, the DISTRICT and the COUNTY, in consideration of the mutual
benefits flowing from each to the other, do hereby agree as follows:
This AMENDMENT NO. 02 shall be effective upon the date of execution by the Parties.
2. The monetary consideration to be provided by the DISTRICT to the COUNTY for the
AGREEMENT is hereby increased by an amount not -to- exceed Three Hundred Eight Dollars
and No Cents ($308,000), for a total revised AGREEMENT amount not to exceed Six Hundred
Fifty Eight Thousand Dollars and No Cents ($658,000).

3. The Payment and Deliverable Schedule is hereby revised in accordance with Exhibit

execute this AMENDMENT NO. 02 on the date first written above.
