A RESOLUTION RENAMING OLDE CYPRESS BOULEVARD
TO LOGAN BOULEVARD NORTH, WHICH STREET IS
LOCATED NORTH OF IMMOKALEE ROAD AND EAST OF 1-75
IN SECTION 21, TOWNSHIP 48 SOUTH, RANGE 26 EAST,
COLLIER COUNTY,FLORIDA.
WHEREAS, the Board of County Commissioners is authorized pursuant to authority of
Chapter 336.05, Florida Statutes, to name or rename streets and roads, except for certain state
roads; and
WHEREAS, Sections Twelve and Thirteen of Ordinance No. 2007-62, codified as
Sections 22-357 and 358 of the Code of Laws and Ordinances of Collier County establishes
criteria and procedures to rename streets or roads;and
WHEREAS, the Board of County Commissioners has been requested to rename Olde
Cypress Boulevard to Logan Boulevard North. This street is located in Section 21, Township 48
South,Range 26 East, Collier County, Florida as depicted on the plat of Olde Cypress,Unit One,
recorded in Plat Book 32, Pages 1 through 11 of the Official.Records of Collier County,Florida.
An illustration of the location of the street is attached hereto as Exhibit"A";and
WHEREAS, the proposed street name is not in conflict with any existing street name;
and
WHEREAS,it is in the best interest of the County to rename this street.
NOW THEREFORE, BE IT RESOLVED BY THE BOARD OF COUNTY
COMMISSIONERS OF COLLIER COUNTY, FLORIDA,that:
The name of this street is hereby changed from Olde Cypress Boulevard to Logan
Boulevard North.
BE IT FURTHER RESOLVED that this Resolution be recorded in the Public Records of
Collier County, Florida and noted upon the maps of the street and zoning atlases of Collier
County and notations made on the referenced Plat.
This Resolution adopted after motion, second and super majority vote this day of

1. Does the document require the chairman's original signature?

3. Original document has been signed/initialed for legal sufficiency. (All documents to be
signed by the Chairman,with the exception of most letters,must be reviewed and signed d
by the Office of the County Attorney.
4. All handwritten strike-through and revisions have been initialed by the County Attorney's N/A
Office and all other parties except the BCC Chairman and the Clerk to the Board
5. The Chairman's signature line date has been entered as the date of BCC approval of the
document or the final negotiated contract date whichever is applicable.
6. "Sign here"tabs are placed on the appropriate pages indicating where the Chairman's
signature and initials are required.
7. In most cases(some contracts are an exception),the original document and this routing slip
should be provided to the County Attorney Office at the time the item is input into SIRE. N/A
Some documents are time sensitive and require forwarding to Tallahassee within a certain
time frame or the BCC's actions are nullified. Be aware of your deadlines!
8. The document was approved by the BCC on January 22, 2013 (enter date)and all
changes made during the meeting have been incorporated in the attached document. b()

WHEREAS, the Board of County Commissioners is authorized pursuant to authority of 5 W
Chapter 336.05, Florida Statutes, to name or rename streets and roads, except for certain state
roads; and

WHEREAS, Sections Twelve and Thirteen of Ordinance No. 2007-62, codified as
Sections 22-357 and 358 of the Code of Laws and Ordinances of Collier County establishes
criteria and procedures to rename streets or roads; and
WHEREAS, the Board of County Commissioners has been requested to rename Olde
Cypress Boulevard to Logan Boulevard North. This street is located in Section 21, Township 48
South, Range 26 East, Collier County, Florida as depicted on the plat of Olde Cypress, Unit One,
recorded in Plat Book 32, Pages 1 through 11 of the Official Records of Collier County, Florida.
An illustration of the location of the street is attached hereto as Exhibit"A"; and
WHEREAS, the proposed street name is not in conflict with any existing street name;
and
WHEREAS, it is in the best interest of the County to rename this street.
NOW THEREFORE, BE IT RESOLVED BY THE BOARD OF COUNTY
COMMISSIONERS OF COLLIER COUNTY, FLORIDA, that:
The name of this street is hereby changed from Olde Cypress Boulevard to Logan
Boulevard North.
BE IT FURTHER RESOLVED that this Resolution be recorded in the Public Records of
Collier County, Florida and noted upon the maps of the street and zoning atlases of Collier
County and notations made on the referenced Plat.
This Resolution adopted after motion, second and super majority vote this c o day of
