EXECUTIVE SUMMARY
Recommendation to approve and authorize the Chairman to sign an amendment to a
Subrecipient Agreement ("Agreement") for David Lawrence Mental Health Center, Inc. in the
amount of $93,000.00 using 2009-2010 Department of Housing and Urban Development (HUD)
Community Development Block Grant (CDBG) funds. This amendment will extend the term
of the Agreement and allow payment for psychiatric services.
OBJECTIVE: To provide an amendment to a Subrecipient Agreement between David Lawrence
Mental Health Center, Inc. and Collier County in the amount of $93,000.00 for rnental health
counseling services. This amendment will extend the term of the Agreement and allow payment
for psychiatric services.
CONSIDERATIONS: Housing, Human and Veteran Services (HHVS) proposes to provide an
amendment to a sub-recipient agreement between Collier County and David Lawrence Mental
Health Services, Inc signed on September 15, 2009 (Item 16D-9) in the amount of $93,000.00 for
mental health counseling services.
On April 28, 2009 the Board of County Commissioners adopted Resolution No. 2009-116
approving the submission of the One-Year Action Plan for FY 2009 - 2010 to HUD. This Action
Plan provided a summary of the projects to be funded with 2009-2010 HUD entitlement funds. The
project included in this agreement was approved as part of the FY 2009-2010 Action Plan.
The proposed amendment will allow an extension ofthe term of Agreement, which originally began
on July 1,2009 and end on June 15, 2010. On May 27, 2010, the Agreement was extended with a
Zero Dollar Modification until August 9, 2010. The amendment will grant a second extension until
December 31, 2010 in order to allow all the CDBG funds to be expended.
In addition, the amendment will extend as well as allow reirnbursement for both contract and
salaried psychiatrists who provide services for David Lawrence Mental Health Center Inc.
Additional non-substantive changes have been made at the request of the Clerk's Office for clarity.
FISCAL IMPACT: No general funds are being utilized in this HUD project.
GROWTH MANAGEMENT IMPACT: This grant will benefit low income residents of Collier
County which is consistent with the goals and objectives of the County's Growth Management Plan.
LEGAL CONSIDERATIONS: This item has been reviewed and approved by the County
Attorney's Office and is legally sufficient-JBW.
RECOMMENDATION: To approve and authorize the Chairman to sign an amendment between
Collier County and David Lawrence Mental Health Inc. using HUD funds that extends the term and
allows payment for psychiatric services.

"David Lawrence Mental Health Center, Inc."
This Amendment, dated , 2010 to the referenced Agreement shall
be by and between the parties to the original Agreement, David Lawrence Mental Health Center, Inc.
(hereafter referred to as "Sub-Recipient") and Collier County, a political subdivision of the State of
Florida, (to be referred to as "County").
Statement of Undentanding
RE: Contract # CD09-0S "David Lawrence Mental Health Center, Inc."
In order to continue the services provided for in the original Agreement document referenced above,
the Sub-Recipient agrees to amend the above referenced Agreement as follows:

AMENDMENT TO: III. TIME OF PERFORMANCE
The effective date of the Agreement between HUD and Collier County shall be July I, 2009.
Subrecipients are authorized to incur eligible expenses after that date and prior to execution of this
Agreement subject to HHS prior written approval. The services of the SUBRECIPIENT shall be
undertaken and completed in light of the purposes of this Agreement. In any event, all services
required hereunder shall be completed by the SUB RECIPIENT prior to JWle IS, 2Q19 December 31.
2010. Any funds not obligated by the expiration date of this Agreement shall automatically revert to
the COUNTY, as set forth in Section XI below.

Any work or services subcontracted by the SUB RECIPIENT shall be specifically by written contract
or agreement, and such subcontracts shall be subject to each provision of this Agreement and

AMENDMENT TO: EXHIBIT "A" SCOPE OF SERVICES
Paragraph A. PROJECT SCOPE and Paragraph B. BUDGET
A. PROJECT SCOPE:
The David Lawrence Center proposes to expand their psychiatric services to uninsured low income
Collier County residents who have been diagnosed with mental illness. This funding will provide a
portion of salary and benefits (50%) of psyehiatrist for multiole David Lawrence osvchiatrist and/or
contracted services at contracted me of $1501hour who will provide psychiatric evaluations/visits,
diagnosis and case management some of which will require follow-up visits to uninsured low income
individuals who have been diagnosed with mental illness. All clients are triaged to determine need
based on routine, urgent, and emergent services. Each of the clients will receive a psychiatric
evaluation from a qualified psychiatrist to detennine the best ongoing course of treatment. All eligible
clients will be required to present documented proof of income in accordance with HUD guidelines.
Collier County Department of Housing and Human Services is funding NINETY THREE
THOUSAND DOLLARS ($93,000) with CDBO funds for this Public Services project.
