Dear Chairman Coyle and Commissioners:
The purpose of the letter is to [I] update you on the progress to date of our" Blue Ribbon Panel"
on Prehospital Emergency Transportation Services {PETS} and [2] to request your funding assistance for
$40,000 to pay for a critical data review and analysis by a Florida Gulf Coast University Professor this
summer so we may use the results when we share with you our Final Report in September, 2010.
Progress to Date:
The state of Pre-hospital care in Collier County is one of dedicated individuals across all services that are
individually providing the best possible care. There is, however. considerable mistrust among PETS
providers due to weak coordination and communication. It is essential that community leaders address
these issues in a civil and forthright process.
The Blue Ribbon Panel first met with the Board of County Commissioners on February 23,20]0 and held
its initial meeting on April 12'" The panel inclnded representatives from hospitals and emergency
department physicians, EMS, Fire, Medical Director and the Sheriff's office. The conclnsions reached
were:
]. A PETS panel should be established and charged with conducting a thorough review ofthe current
state of PETS services provided in Collier County. The panel should oversee the development of
countywide evidence-hased clinical protocols for (BLS) and (AI,S) services (a multi-tiered system), best
practice policies, techniques, standardization of equipment and material, certification and training in
Collier County.
Action: The panel initially met on April 12, 20] 0 with three additional meetings and multiple sub
committee meetings. The panel identified a model which has successfully integrated both EMS and Fire
services on Marco Island. The Marco model is actually a multi-tiered BLS system coordinated with AI,S
as required. The panel also reviewed mulliple evidenced based documents and articles regarding AI,S
and BI,S services, best practices and ontcomes. Based on those reviews, a model for BI,S and AI,S
services was suggested in which BI,S services would be provided within 4 minutes and AI,S services
within 8 minutes. (Please see attachment] ji" list olattendeesj
2. An objective evaluation of the data related to emergency responses in Collier County is critical to
understanding the challenge/problem.
Action: A sub-committee led by Chief James Bloom, CCSO, identified and reported back to the
panel an array of the appropriate data. Following the identification of an analytical expert from FGCU, a
proposal for an in-depth review of the data by said expert was drafted, reviewed by the pancl and
delivered to the County Manager for consideration. (Please see attachment)

We respectfully requests that the Board of County Commissioners approve the $40,000 request for
funding of the PETS Data review to provide us with the information to refine our draft model and finalize
our recommendations on the implementation of a multi-tiered system to be delivered to you in September.
Specifically, the PETS Data study will produce quantifiable and measurable deliverables including but
not limited to the identification of barriers to delivery ofBLS services within countv [boundaries of
various districts; location of vehicles within districts], the location onow, medium and high rate areas. bv
tvpe of call [focusing on the identification of response times by areas/districts and rates, location of
response team. total time spent in response including on scene, transport and return to base and outcome
based on type of call] and the analysis of existing resRYnder capacitv and possible solutions to meet 4-
minute BLS and 8-minute ALS response bv type of cal!.
To the best of our knowledge, this will be the first and most comprehensive, coordinated and centrally
focused, "reality study" of the "system" of PETS in Collier County. The studv results will enhance our
final recommendations to you so you may identify the ~eaknesses and strengths of our delivery svstem
and assist all to understand the potential impact of a communi tv-wide BLS response program.
We appreciate the opportunity you have given us to attempt to serve and respectfully ask for your
affirmative support for our request so we may complete and present 10 you our report in September so you
may advocate steps 10 positively contribute to PETS improvements in Collier County.

Re: Scope of EMS Data Evaluation project
Dear Mr. Ochs,
The Pre-hospital care Blue Ribbon Panel requests an objective evaluation of the data
related to emergency responses in Collier County which is critical to understanding the
challenge/problem, Absent a thorough review of the data, it is ill advised for anyone to
draw conclusions as respects the best design of a comprehensive EMS delivery model for
the entire county. The Committee strongly suggests that the panel oversees an objective
review of the data, said review to be conducted by an objectivc outside authority. Once
the emergency response data has bccn compiled and appropriately analyzed, the panel
will gain valuable insight which should lead to changes that will improve the
effectiveness and efficiency of EMS services Countywidc.
Proposal :
I. Discuss with dispatch management the availability, issues and reliability of
data sources,
a. Sub-committee established through Sheriffs office to provide data
b, Draft report generated by sub-committee of EMS/Fire calls for Collier
County
c, Data points approved by Pre-hospital care panel (see attached detail data
report and points)
i, Day, time and types of calls
ii. Origin and disposition of calls
iii. Who responds and timeliness of response
iv. Vehicles and vehicle origin responding to calls
v. Accuracy of data points from EMS, Fire and Sheriffs Office
vi. Hospital patient outcome
vii. Community response and AED use
2. Verify that identified specific data will provide pertinent information
regarding key thresholds in the delivery of pre and post hospital care.
