EXECUTIVE SUMMARY
Recommendation to approve an Amendment to Agreement authorizing a rate increase to
$80 per boarding for Stretcher Medicaid trips for the paratransit services provided under
Collier County's Agreement with McDonald Transit Associates, Inc. d/b/a Collier Area
Transit ("McDonald Transit")."
OBJECTIVE: Recommendation to approve an Amendment to Agreement to the September] 5,
2009, Agreement between McDonald Transit and Collier County, pertaining to the provision of
fixed route and paratransit system service and support, to allow for a ra1e increase to $80 per
boarding for the subcontractor{ s) providing stretcher Medicaid trips for paratransit services under
McDonald Transit Associates, Inc.
CONSIDERATIONS: Collier County's contract with McDonald Transi1 Management, Inc.
(Consolidated Management Contract for the Collier County Fixed Route and Paratransit Systems
- Contract #05-3845) permits the use of private subcontractors to provide paratransit services
subject to the prior written approval of the County.
On November 12, 2009, the sole qualified subcontractor provided the required 90 days notice to
McDonald Transit Associates, Inc., specifying its intent to terminate service of non-emergency
stretcher trips due to escalating costs. The fare for non-emergency stretcher trips include a
boarding fee plus a rate per mile. For example, under the current rate schedule the charge for a
ten-mile trip in Collier County would be $43.46 ($27.56 for the boarding fee plus $15.90 for the
mileage). The subcontractor requested an increase in the boarding fee to $80 per boarding and
did not request an increase in the rate charge per mile. Upon notification to McDonald Transit,
McDonald commenced a solicitation to obtain alternative vendors that resulted in two
unfavorable responses. Additionally, McDonald Transit performed an evaluation of the industIy
rates for non-emergency stretcher services and concluded that Collier County's rates were more
than 60% less than what is being charged within the surrounding areas.
Non-emergency stretcher service represents only I % of the paratransit service but is a needed
service within this community, This service is a specialized market because there are not many
vendors that have the equipment required to perform the service.
Based on the market and the special ized service, it is recommended that the rate for non-
emergency stretcher trip be increased to $80 per boarding and the per mile rate remain at $1.59
per mile, Even though the proposed rate increase is a significant increase from the current
boarding rate of $27.56, it is still $40.00 less than either Lee or Hendry County's in county
boarding rate ($120.55) and $50.00 less than either of their Out of County rate ($130.55),
Further, while Collier County's mileage charge for such transports remains at $1.59 per mile,
Lee and Hendry County charge at a rate of $7.79 per mile in addition to their higher boarding
fee. The last rate increase for paratransit service implemented by the County was in Sep1ember
2007.

FISCAL IMPACT: The fiscal impact associated with this approval will have to be
incorporated into the agreement between Collier County and McDonald Transit Associates, Inc,
The additional cost associated with this amendment will be absorbed within the current operating
budget in CAT Transit Enhancement Fund 426 project 61011. This increase in cost was
approved by the LCB at its 3/10/10 meeting.
LEGAL CONSIDERATIONS: This item has been reviewed and approved by the County
Attorney's Office and is legally sufficient for Board action-SRT.
GROWTH MANAGEMENT IMPACT: There is no Growth Management Impact associated
with this Executive Summary. The approval of this request will be consistent with Objectives 10
and 12 of the Transportation Element of the Growth Management Plan.
RECOMMENDATION: That the Collier County Board of Commissioners approve the
Amendment to Agreement providing for a rate increase of $80 per boarding for the
subcontractor(s) providing Stretcher Medicaid trips for the paratransit services under McDonald
Transit Associates, Inc. and authorize its Chairman to sign the Amendment.

Analysis of Stretcher Rate and Service
McDonald Transit Associates, Inc. has a contract with TLC to provide paraTransit service,
including stretcher service. That contract was recently renewed on October 1, 2009. Section
13 - Termination without cause, of the contract provides that contractor may terminate service
with 90 days written notice. Written notice was provided by TLC on November 12, 2009. Based
on that date, service would end on February 9, 2010.
McDonald Transit Associates, Inc. conducted a solicitation for vendors to provide non-
emergency stretcher service for CAT. The solicitation was completed in December,

AMENDMENT TO AGREEMENT
McDONALD TRANSIT ASSOCIATES, INC.
THIS AMENDMENT TO AGREEMENT ("Amendment") is made and entered into this
day of ,2010, by and between McDonald Transit Associates, Inc.,
whose address is 3800 Sandshell Drive, Suite 75, Fort Worth, Texas 76137 ("Contractor") and
The Board of County Commissioners of Collier County, Florida, as the governing body of
Collier County ("County").
RE C I TAL S:
WHEREAS, the parties entered into an Agreement dated September 15, 2009
("Agreement"), wherein the Contractor agreed to provide Transit System Service and Support to
Collier County for the Collier County Fixed Route and Paratransit Systems route operated as
Collier Area Transit ("CAT''); and
WHEREAS, the sole qualified subcontractor providing stretcher Medicaid trips for the
paratransit services for the Contractor has provided notice of its intent to terminate providing
non-emergency stretcher trips due to escalating costs; and
WHEREAS, an evaluation of industry rates for non-emergency stretcher service revealed
that the rates for providing such services in the current contract are more than 60 % less than that
being charged in the surrounding service areas; and
WHEREAS, the parties wish to modify Attachment 3 of the Agreement to reflect a
desired increase to the Boarding Rate for Paratransit stretcher services under the Agreement; and
WHEREAS, upon recommendation by the Transportation Administrator and after
reasoned consideration by the Board of Commissioners, the Board finds that this Amendment
enhances the County's transportation system and is in the public interest.
WITNESSETH:
NOW, THEREFORE, in consideration of Ten Dollars ($10.00) and other good and
valuable consideration exchanged amongst the parties, and in consideration of the covenants
contained above and herein, and the fact that all of the above recitals are true and correct and
shall be fully incorporated herein and form part of the basis for this Amendment, the parties
agree as follows:
1. The Boarding Rate for In County and Out of County Vendor Rates currently set forth
in the Agreement as $27.56 is hereby amended and increased to $80.00 per boarding. The rate
per mile of $1.59 shall remain the same.
2. There are no other amendments to this Agreement except those set forth herein. This
Amendment shall only be amended or cancelled by mutual written consent of the parties hereto
or by their successors in interest.
