EXECUTIVE SUMMARY
Recommendation to approve and authorize the Chairman to sign a contract amendment between
Collier County Board of County Commissioners and the Area Agency on Aging of Southwest
Florida, Inc., dba Senior Choices of Southwest Florida and approve a budget summary revision
that reflects a level transfer of grant funding in the amount of $5,172 between budget categories for
the FY 2011/12 Home Care for the Elderly (HCE) Program.
OBJECTIVE: Recognize an adjustment between two categories of grant funding for the Services for
Seniors program in the amount of $5,172.00 effective December 1,2011, with no adjustment to the total
grant amount.
CONSIDERATIONS: Housing, Human and Veteran Services Department Services for Seniors program
has been providing support services to Collier County's frail, elderly citizens for over 30 years through the
Home Care for the Elderly (HCE) program. The current amount of the HCE grant is $83,391.00 which
was originally approved by the Board July 26, 2011, item 16D12.
This grant funding is awarded by the State of Florida Department of Elder Affairs (DOEA), and managed
by the Area Agency on Aging of Southwest Florida, Inc., dba Senior Choices of Southwest Florida.
Senior Choices frequently makes adjustments (many very small) to funding within the various program
components to best match the needs of the particular clients being serviced. It is impossible to know the
exact needs of clients until they are qualified as program participants and care plans are established. Once
the needs are known, then Senior Choices adjusts grant funds accordingly. In this particular case, the
DOEA is making a level transfer of $5,172.00 between two similar categories of grant funds in order to
better match services to those particular clients being served. Their method for recording these changes is
a contract amendment which requires Board of County Commissioners approval.
FISCAL IMPACT: Since the Home Care for the Elderly (HCE) total budget amount of $83,391.00 will
remain the same, it will not impact the County budget. Funding for the HCE FY 2011/12 program is
available within the Human Services Grant Fund (707), Project 33162.
LEGAL CONSIDERATIONS: The Amendment and Attestation Statement are form documents
provided by the Area Agency on Aging of Southwest Florida dba Seniors Choices of Southwest Florida.
The Attestation Certification certifies that no substantive changes were made to the forms. This item is
legally sufficient and requires a majority vote for Board action. -JBW
GROWTH MANAGEMENT IMPACT: There is no growth management impact from this budget
revision.
RECOMMENDATION: To approve and authorize the Chairman to sign an Area Agency on Aging for
Southwest Florida, Inc., dba Senior Choices of Southwest Florida budget revision for the Home Care for
the Elderly (HCE) program and approve the budget revision to reflect a level transfer of $5,172 between
two program components.

This AMENDMENT, entered into by the Area Agency on Aging for Southwest Florida, Inc., dba
Senior Choices of Southwest Florida hereinafter referred to as the "Agency ", and Collier
County Board of County Commissioners, hereinafter referred to as the "Recipient ", amends
agreement #HCE 203.11.
The purpose of this amendment is to revise Attachment VII, Budget Summary. The revised
2011 -2012 allocation is shown below with a decrease of $5,172.00 of HCE Basic Subsidies to an
increase of $5,172.00 in HCE Special Subsidies.

This amendment and all of its attachments are hereby made a part of this agreement.
