^ EXECUTIVE SUMMARY
Recommendation to approve and summing the Chairman to sign Signature Authority Forms for
three (3) Disaster Recovery Initiative(DRI) Grant agreements between the Florida Department
of Community Affairs and Collier County. These forms will update Collier County's Contact
Person, Financial Contact Person, and designme individuals unmarked to sign Real for Funds
(BIT) for each of these groats.
OBJECTIVE, Recommendation to approve and authorize the Chairman to sign Signature Authority
Forms for Disaster Recovery Initiative(DRI) Coot agreements between the Florida Department of
Community Al and Collier County. These forms will update Collier Cowry's Contact Person,
Financial Contact Person, and designate individuals authorized m sign Request for Ponds (RFF) for each
of these grants.
CONSIDERATION- The Board of County Commissioners accepted three fedemlly -Ponded Disaster
Recovery Initiative grant agreements with the Florida Department of Community Affairs . Several
hurricane hardening projects were established for binding under these agreements.
The Florida Department of Community Affairs requires submission of updated Signature Authority
Forms in the event of changes in the County's staff administermag the gams, Due fo persomel changes
in the Department of Housing Human and Veteran Services (HHVS), designated contacts involved In
the administration of these Cones have changed. Submittal of these forms will provide the DCA with an

The signature authority approval form for grant X101113- 114- 09- 21- 01 -K09 as pan ofthe grant agreement
approved by the BCC on May 25, 2010 was erroneously numbered and corrected on this updated form.
Approval ofthls item will update Collier County's Signature Authority Forms with the Florida
Department ofCommunity Affairs and accurately identify the current Housing, Human and Veteran
Services Department staff involved in the administration ofthese federally funded disaster grants.
FISCAL IMPACT, These forms have zero impact on the approved budgets for these gods and their
projects. No general hinds are associated with these projects.
GROWTH MANAGEMENT IMPACT There are no growth management impacts associated with
thisprogem.
LEGAL CONSIDERATION: This item is legally sufficient and requires a majority vote for Board
