OBJECTIVlt: To receive after-the-fact approval from the Board of County Commissioners to submit an Assistance to Firefighters Grant (AFG) application to-the Federal Emergency Management Agency (FEMA) to fund the purchase of three replacement ambulances for Emergency Medical Services. "' CONSIDERATIONS: On August 30,2011, the County Manager approved the electronic submittal of an AFG grant application, in accordance with Collier County CMA #5330 which authorizes the County Manager,to approve the ,submittal of grant applications with subsequent BCC action at the next available Board meeting to ~tify the approval as "after-the-fact." The Assistance to Firefighters Grant program typically provides only a month for development and submittal of ~t applications. This year, the program was announceci on August 15th with a deadline, of September 9 . This deadline was subsequently extended to September 16th. There was no Board Meeting held during the month of August. This grant was submitted by EMS to request funding to purchase three ambulances that are on the Fleet Replacement Schedule, but with ongoing budget constraints, there has been no funding for their r-.. replacement. The total amount of funds requested for this project is $560,514, and comprised of $448,412 to be funded by the grant and a match $112,102 (20%) to be funded by the County. If awarded, staff will come back to the Board to request approval to accept the grant. ' FISCAL IMPACT: There is no Fiscal Impact at this time. LEGAL CONSIDERATIONS: The Board will have the opportunity to acceptor reject the funds if the grant is approved. Accordingly, this Office has no issue with respect to the legal sufficiency of this request, which is appropriate for Board action. This is a regular item requiring a majority vote. -JBW GROWTH IMPACT: There is .no Growth Management Impact resulting from this' action. The grant request will be $448,411 and, if awarded, EMS would provide a 20% match of $112,103 towards the total $560,514 necessary to fund the purchase of these trucks. The remaining 20% is utilized to cover operating costs and capital purchases to replace worn equipment. The budget is largely dependent (40%) on ambulance fee revenues. With the majority of the county being agricultural, there is a large influx of both a migrant population and indigent patients. Especially significant is a referendum in the State of Florida to reduce property taxes that has crippled county agencies from obtaining necessary funding through ad valorem taxes. In addition, and common throughout the United States, property values have tumbled and Collier County has experienced an $86 million loss in revenue since FY08. We are unable to apply to the State of Florida for grants because only projects for new or expanded services are eligible under that program. Protect Dcscrintion: This grant request is respectfully stlbmitted to provide $448,411 for three medium duty ambulances comprised of Ford F550 chassis and a HOlion Box. Collier County will match 20% ($112,103) for a total of$560,514 to fund the purchase of this equipment. These vehicles would be purchased through the "Florida She1'iffs Bid for Fire Rescue Vehicles and Other Fleet Equipment.1I Safety of our crews) patients and the general public are first and foremost. In June 2011 one of our trucks expel'ienced a complete electrical failure during a transport. Not only did the truck have to be towed to motor pool for repair, but another ambulance had to be summoned to resume the transport of the patient. The next day) this same truck was again transporting a patient when the engine falied. On.ce again, delay in transport and another ambulance taken from its zone to continue the transport. Recently another truck experienced a power failure (again during transpOli) and was out of service for five days. Breakdowns are particularly problematic for us because our County is so large and transports often initiate in very remote areas, many miles from our local hospitals. Breakdowns of any type can have adverse outcomes fol' cddcally ill 01' injured patients. We are especially sensitive to this danger because a truck broke down a few years ago while transporting a patient from a l'emote location and although a replacement unit was dispatched and completed the patient transport) she eventually died from injuries she had suffered. In addition to the patient risks, breakdowns affect coverage in other zones as another unit is dispatched to replace a broken~down ambulance. Becallse our crews are spread so thin and often have long waits at hospitals) p~U'ticularly during the heavy visitors' season, any equipment failm'e puts a strain on the entire system and consequently the public may suffer slower response times to emergencies. In addition) there is a liability to the County for worn Ol.lt or unsafe equipment. ~ Collier County is served by 83 miles of Intel'state 75 (Alligator Alley). Approximately 68 of these miles are crossing the state through national preserves and everglades swamplands, According to the Florida Department ofTl'ansportation, from 2008 to 2010, the annual average traffic count went from 9 million to 9.5 million vehicles. During that time period, Collier County EMS responded to about 3600 calls per year on that highway system, with about 350 per year in the remote areas ofI-75 between Collier and Bl'OWal'd counties, Cost! Benefit: Collier EMS cU1'rently has a fleet of33 ambulances, In 2007 we began pm'chasing HOlton ambulances with boxes that are bt:tter quality and more rugged to allow them to be remo.unted on new chassis at least once; in effect dOl.lbling the lifespan of the box and reducing the replacement costs of new ambulances. The remounting process is scheduled to begin in FY14. The County's Fleet Management Department has recommended replacement of amb\dances each year, but the department has been unable to fund these replacements due to budget constraints. The department currently has ten old and high mileage ambulances that are recommended fOl' replacement. The three highest mileage trucks have in excess of 250,000 miles; two of them are actually closer to 270,000 miles. In addition, another of the ten has been deadM lined due to engine failure. During the three year period, 2008 - 2010) our fleet averaged approximately 700,000 miles per year. Because ambulances ~u'e normally idling for long periods of time on scene and at the hospitals, there is even greater wear and tear on the engines than the mileage reflects, One source indicates that 60 minutes ofidHng time is equivalent to between 80 and 120 minutes of driving time, Also) because aftheir age and type) these trucks are not suitable for the remount plan. Even iftl1is had not been the case, the welds on the boxes are broken and their general condition does not make them conducive to a remount plan. Cost, of course) is always a factor. Over the past several years, maintenance and repair costs have skyrocketed. It has become a financial burden keeping these old units operational. All of the trucks have exceeded 1heir cost in maintel1ance and repair costs, some by twice their purchase price. In additiOll, repair and maintenance resoUl'ces that are being spent on these units could be better utilized on other more viable vehicles, Should this grant not be funded, Collier EMS is in jeopardy of not having sufficient transpOl't vehicles to keep up with our call volume, postponing treatment and transports) which may ultimately result in negative outcomes for OUl' patients, Statement of Effeet: If awarded this grant through the assistance of the U. S. Depat'tment of Homeland Security) Collier County EMS wiII be able to aSSUl'e that our citizens and visitors have reliable transport vehicles to achieve acceptable response times and favorable patient outcomes. Even though our Fleet Department has recommended a replacement schedule for our ambulances, we have been unable to accomplish this goal due to budget constmints. OUI' capital budget to purchase worn out medical equipment has been completely eliminated for FY 2012 and the operating budget has been cut for fOlll' consecutive years. No pay raises have been given, In fact, several years ago the union members voted to not take a scheduled COLA adjl.lstment that was contractually obligated in an effort to avoid layoffs which would have OCCUlTed due to the budget constraints. The safety of our crews and patients is being threatened by vehicles that are experiencing increased mechanical failures due to age and wear and teal'. The award of this grant will allow us to remove the most problematic of 01lI' fleet and replace them with reliable vehicles that will last twice as long due to our remount program) improve l'esponse times and provide safe and reliable transports for our patients.
