11� EXECUTIVE SUMMARY
Recommendation to approve an after- the -fact amendment between Collier County and the Area
Agency on Aging for Southwest Florida, Inc. d/b /a Senior Choices of Southwest Florida to reflect a
change in the Home Care for the Elderly (HCE) grant program.
OBJECTIVE: The execution of this agreement and budget amendment is necessary for uninterrupted
support services to Collier County Services for Seniors (CCSS) frail, elderly clients.
CONSIDERATIONS: An amendment to the HCE contact was received from the Area Agency for
Southwest Florida, Inc. d/b /a Senior Choices of Southwest Florida requiring that as of October 16, 2012,
Collier County as lead agency pay vendors directly for in -home services and supplies. Accordingly, a
portion of the Special Subsidies budget is being allocated to Collier County Services for Seniors (CCSS)
based on current grant expenditures and projections.
The amendment to the HCE program is being presented as after -the -fact because Collier County received
/" *N the grant amendment on October 18, 2012 and we did not learn that immediate execution was required
until after the agenda deadline for the November 13, 2012 Board meeting had past. Consequently,
pursuant to CMA 5330 and Resolution No. 2010 -122, the County Manager authorized Steve Carnell,
Interim Administrator, to sign the contract amendment. Collier County as the Lead Agency is responsible
to respond to seniors needs and to manage the spending authority for the HCE program services.

FISCAL IMPACT: A budget amendment is required for Collier County to pay vendors for services
through June 30, 2013 in Human Services Grant Fund (707), Project 33217. No match is required.
Funding in the amount of $14,508 from Special Subsidies is being allocated for vendor payment of in-
home services and supplies. The balance remaining is for caregiver reimbursement directly from Senior
Choices. Senior Choices previously made all payments directory to the vendors and caregivers and
Collier County was only reimbursed for Case Management.
GROWTH MANAGEMENT: There is no growth management impact from this recommendation.
LEGAL CONSIDERATIONS: This is a standard form amendment provided by Senior Choices of
Southwest Florida. This item is legally sufficient and requires a majority vote. — JBW
RECOMMENDATIONS: That the board approve an after - the -fact approval amendment and attestation
statement between Collier County and the Area Agency on Aging for Southwest Florida, Inc. d/b /a Senior
Choices of Southwest Florida and authorize the budget amendment.

STANDARD CONTRACT
HOME CARE FOR THE ELDERLY PROGRAM
COLLIER COUNTY BOARD OF COUNTY COMMISSIONERS
THIS AMENDMENT is entered into between the Area Agency on Aging for Southwest Florida, Inc. dba Senior Choices
of Southwest Florida ( "Agency ") and Collier County Board of County Commissioners, ( "Recipient "), amends agreement
HCE 203.12.
The purpose of this amendment is to add Item 3.1.3 under ATTACHMENT I, STATEMENT OF WORK, SECTION
III: METHOD OF PAYMENT and add additional rates on ATTACHMENT VIII, BUDGET AND RATE
SUMMARY.
SECTION III: METHOD OF PAYMENT
3.1.3 Effective October 16, 2012, the Recipient will be responsible for paying vendors for in -home services. The
Agency will reimburse the Recipient for these services based on the contracted rate, ATTACHMENT VIII,
BUDGET AND RATE SUMMARY.
This amendment shall be effective on October 16, 2012. All provisions in the agreement and any attachments thereto in
- onflict with this amendment shall be and are hereby changed to conform with this amendment.
All provisions not in conflict with this amendment are still in effect and are to be performed at the level specified in the
agreement.
This amendment and all of its attachments are hereby made a part of this agreement.
