Agenda 09/11/2012 Item #16D14Recommendation to approve four (4) amendments Ifor the FY12 agreements and attestation statements with the Area Agency on Aging of Southwest Florida, Inc., dba, Senior Choices of Southwest Florida, with fiscal impact of $9,000 (decrease in funding) for the Nutrition Services Incentive Program. OBJECTIVE: To receive approval from the Board �f County Commissioners (BCC) for minor and routine contract and budget amendments to the Community Care for the Elderly (CCE), Home Care for the Elderly (HCE), Alzheimer's disease Initiative Program (ADI) and Nutrition Services Incentive Program (NSIP) grants. CONSIDERATIONS: The Services for Seniors I and Veteran Services Department (HHVS), has 1 County's frail, elderly citizens for over 32 years programs. These grants are received from the St, (DOEA) and managed by Senior Choices of Southc contracts. Senior Choices, from time to time, will match the needs of clients. The CCE, HCE, and ADI grant contracts were awar and commenced in July 1, 2012. On July 24, 2012, on the case management rate in these three contracts. proposed to be revised from $55.50 to $55.56 in effective dates of Julv 1. 2012. The amendment for the NSIP contract with Senior Choices is associated with one (1) budget amendment reflecting a decrease in funding of $9,00 . FISCAL IMPACT: The only fiscal impact is to the NSIP grant program. The allocation for NSIP grant program resides in Fund 707, Human Se ices Grants, FYI 1-12 NSIP project 33182. This allocation is being reduced by $9,000.00 through a budget amendment. There is no match requirement, and no fiscal impact to General Funds. ' LEGAL CONSIDERATIONS: The proposed amendments are proposed to be effective July 1, 2012 for the CCE, HCE, and ADI programs and August 9, 2012 for NSIP program. In the event of conflict in language among any of the documents referenced above, the specific provisions and requirements of the contract document(s) shall prevail over inconsistent provisions in the proposal(s) or other general materials not specific to this contract document and identified attachments. 3. Term of Contract This is a two -year contract shall begin on July 1, 2012 or on the date on which the contract has been signed by the last party required to sign it, whichever is later. Renewal By mutual agreement of the Parties, in accordance with s. 287.058(1)(g), F.S., the Agency may renew the contract for a period not to exceed three years, or the term of the original contract, whichever is longer. The renewal price, or method for determining a renewal price, is set forth in the bid, proposal, or reply. No other costs for the renewal may be charged.
