EXECUTIVE SUMMARY
Recommendation to approve selection committee rankings and enter into negotiations with
the top ranked firm, Victor J. Latavish, P.A., for a contract for Request for Proposal No.
12 -5923, "Professional Design Services - Eagles Lakes Community and Fitness Center,"
Project Number 80184.
OBJECTIVE: To provide professional design services for the Eagles Lakes Community and
Fitness Center located at 11565 Tamiami Trail E.
CONSIDERATIONS: The Parks and Recreation Department is planning to expand the Eagle
Lakes Community Park to include a community and fitness center as well as a future pool
complex. The park built in 1999 is located at 11565 Tamiami Trail East and currently offers a
variety of outdoor recreational areas as well as open green space. The proposed community and
fitness facility, a building approximately 8,000 to 10,000 square feet will include: office space,
community room(s), kitchen, restrooms, mechanical room, janitors' closet(s), a fitness room and
a locker room. The construction of this facility will allow access to the future pool complex with
additional locker rooms, restrooms, showers and a lifeguard room. The services of a design
professional are required to continue with this project.
Request for Proposal (RFP) No. 12 -5923, Professional Design Services — Eagle Lakes
Community and Fitness Center, was issued in accordance with Florida Statute 287.055,
Consultants' Competitive Negotiation Act, and posted on June 14, 2012, with 1,189 notices sent
and 72 firms downloading full document packages. By the July 6, 2012, deadline, eight (8)
responsive proposals were received. RFP 12 -5923 will provide professional design services
including, but not limited to, permitting, specifications, construction field observation,
management of construction documentation, and post construction services as needed. The
selection committee met on July 13, 2012, and by consensus selected Victor J. Latavish, P.A. to
provide the required services. The final rankings are shown below.

Victor J. Latavish, P.A. is qualified and has experience both designing and inspecting various
design projects. Staff recommends approval of the short list and authorization to commence
contract negotiations with the top ranked firm, Victor J. Latavish. After contract terms are
agreed upon, staff will return to the Board for final approval of the proposed contract.

FISCAL IMPACT: There is sufficient funding available in the Eagle Lakes project 80184 to
negotiate this contract.
Once the design phase is complete and construction finalized; the estimated expenses to maintain
park operations are shows in the table below.

LEGAL CONSIDERATIONS: This item has been reviewed by the County Attorney's office,
is legally sufficient, and requires majority vote for approval. -CMG
GROWTH MANAGEMENT IMPACT: This project is consistent with the Recreation and
Open Space Element of the Growth Management Plan.
RECOMMENDATIONS: That the Board of County Commissioners approve the selection
committee rankings and authorize entering into negotiations with Victor J. Latavish for a
contract for Request for Proposal No. 12 -5923, "Professional Design Services — Eagle Lakes
Community and Fitness Center," Project Number 80184. In the event that agreement cannot be
reached with the top - ranked firm, the Board authorizes negotiation in accordance with Florida
Statute 287.055.
