EXECUTIVE SUMMARY
Recommendation to approve Amendment No. 2, changing Exhibit A, Project Scope of an existing
Subrecipient Agreement with Goodwill Industries of Southwest Florida, Inc. in order to utilize all
of the Community Development Block Grant (CDBG) funding for completed renovations at the
Roberts Center in Immokalee, Florida.
OBJECTIVE: Provide for changes in the project scope of an existing Community Development Block
Grant (CDBG) agreement with Goodwill in order for Goodwill to address the most urgent needs at the
Roberts Center in Immokalee, Florida.
CONSIDERATIONS: On October 26, 2010, item 16D3, a subrecipient agreement was approved by the
BCC, for the 2010 -2011 CDBG Roberts Center Renovation project to Goodwill.
An amendment is required to update language in the project scope of the existing agreement.
Specifically, kitchen renovations are being removed from the scope of work in order to allow Goodwill to
address more urgent needs at the Roberts Center. This change in scope allows for the following
completed items: i.e. complete remodeling of restrooms, hurricane hardening, installation of an exterior
canopy, HVAC renovations, interior and exterior painting, construction demolition and removal, exterior
concrete walkways, insulation, electrical, lighting, acoustical ceiling, strip /clean flooring, permit fees,
asbestos report, field supervision, carpentry and materials, insulation of new offices, doors and trim, fire
extinguishers, voice and data wiring, and creation of new offices. This change will not affect the original
total budget amount.
FISCAL IMPACT: No general funds are associated with this CDBG Subrecipient amendment. There
is no fiscal impact associated with this amendment. 2010 -2011 CDBG Funds are located within Housing
Grants Fund (705), Funded Program 33088.9.
GROWTH MANAGEMENT IMPACT: The CDBG project associated with the amendment to the
subrecipient agreement will help meet the goals, objectives and policies set forth in the Housing Element
of the Growth Management Plan.
LEGAL CONSIDERATIONS: This item is legally sufficient and requires a majority vote for Board
action. - JBW
RECOMMENDATION: To approve Amendment No. 2, changing Exhibit A, Project Scope to the
2010 -2011 CDBG subrecipient agreement with Goodwill Industries of SW Florida, Inc. and authorize the
Chairman to sign same.

THIS AMENDMENT, dated 2012 to the above - referenced
Agreement shall be by and between the parties to the original Agreement, between Goodwill Industries
of Southwest Florida, Inc.," a private not - for - profit corporation existing under the laws of the State of
Florida, having its principal office at 4940 Bayline Drive, North Fort Myers, FL 33917, and its Federal
Tax Identification number as 59- 6196141 and DUNS #, 152823662, ( "SUBRECIPIENT "), and Collier
County, a political subdivision of the State of Florida, (to be referred to as "COUNTY. ")
RE: Contract HUD Grant #B- 10 -UC -12 -0016 CD10 -07, IDIS #380, "Goodwill Industries of SW
Florida, Inc.- Roberts Center Renovation Project."
The following change to the above referenced Agreement has been mutually agreed to by the
Subrecipient and the County. The additions to the existing language in the Agreement are shown

SCOPE OF SERVICES
GOODWILL INDUSTRIES OF SW FLORIDA, INC.
ROBERT'S CENTER RENOVATION PROJECT
A. PROJECT SCOPE:
On April 27, 2010, the BCC approved the HUD Action Plan allocating funding for this
initiative. Community Development Block Grant (CDBG) funding will be used to renovate the
Robert's Center.
In the Action Plan for this program the intended outcome of this funding is to provide
rehabilitation and upgrades to the Roberts Center. The renovation activities will may include
interior /exterior renovations including office build outs, and hurricane hardening and
Renovation activities /items /services may include but not be limited to, demolition, repair
and/or replacement of the following: roofs, floors, ceiling, walls, structural items, doors,
windows, HVAC, electrical, plumbing systems, fixtures, cabinetry, appliances, and ramp for
wheelchair accessibility and exterior improvements as well as associated labor and supervision.

The detailed project scope will be contained in the schedule of values awarded the project's
construction contract. The project construction contract will include details sufficient to
document the number, amount, and costs associated with all activities.
The project activities will meet the U.S. Department of Housing and Urban Development's
national objective to benefit low and moderate income persons and improve the
quality /quantity of neighborhood facilities for low - income persons.
B. BUDGET:,
Collier County Housing, Human and Veteran Services is providing One Hundred Seventy Four
Thousand, Five Hundred Three Dollars And Sixty Cents ($174,503.60) in CDBG funding for
the project scope described above.
