Withdraw Item 11G: Recommendation to award RFP #11 -5785 Management of Pelican Bay
Tennis Center and authorize the Chairman to sign a contract with DBA /The Naples Tennis
Academy. (Staffs request due to receipt of written objection to award)
Withdraw Item 14A1: Recommendation to authorize the County Attorney to take all
necessary action, including filing a lawsuit, to evict Three Mayhoods, LLC from its present
location at the Immokalee Regional Airport, and to pursue any holdover rent, damages, and
costs that may be due and owing to the Airport Authority. (Staffs request due to receiving
signed copy of lease agreement)
Move Item 16A18 to Item 11N: Recommendation to approve a Settlement Agreement and
Release with Malcolm Pirnie, Inc., to settle all outstanding claims related to consulting
services provided in connection with project and oversight services on Forest Lakes MSTU
Projects F -53 and F -56, and to authorize payment. (Commissioner Hiller's request)
Move Item 16131 to Item 14133: Recommendation that the Collier County Community
Redevelopment Agency review and approve the Bayshore Gateway Triangle and Immokalee
CRA's 2011 Annual Reports, forward the reports to the Board of County Commissioners and
Clerk of Courts and publish public notice of the filing. (Commissioner Hiller's request)
Move Item 16132 to Item 14134: Recommendation that the Collier County Community
Redevelopment Agency (CRA) authorize the CRA Director to take appropriate steps to sell
twenty-three (23) CRA owned vacant lots within the Bayshore /Gateway Triangle Mixed Use
District Overlay in accordance with Florida Statute 163.380 and bring back any offers
deemed to meet the terms of the proposal for review and approval. (Commissioner Hiller's
request)
Move Item 16C1 to Item 11L: Recommendation to reject all bids received in response to
Invitation To Bid #11 -5624 for Trenchless Sewer System Rehabilitation Services.

Move Item 16D8 to Item 11M: Recommendation to approve a substantial amendment to
the Collier County 2011 -2012 Department of Housing and Urban Development (HUD) Action
Plan. The amendment will allow Community Development Block Grant (CDBG) funds in the
amount of $775,016 to be re- programmed from Eagle Lakes Park to the Immokalee South

EXECUTIVE SUMMARY
Recommendation that the Board of County Commissioners, acting as the Collier County Airport
Authority, approve the attached Advertisement License Agreement with Seminole Gaming
Management, LLC for advertising at the Immokalee Regional Airport.
OBJECTIVE: To obtain approval of an Advertisement License Agreement to authorize
Seminole Gaming Management, LCC to advertise at the Immokalee Regional Airport.
CONSIDERATIONS: Seminole Gaming Management, LLC desires to enter into an advertising
license agreement with the Collier County Airport Authority to advertise on one (1) window at
the Immokalee Regional Airport.
Airport Management and Seminole Gaming Management have agreed to an advertising lease
rate of one hundred sixty two dollars ($162) per month plus applicable Florida sales tax, payable
twelve (12) months in advance, to advertise on one (1) window at the Immokalee Regional
Airport.
FISCAL IMPACT: Additional revenue totaling $1,944 will be received within Collier County
Airport Authority Fund (495) benefiting Immokalee Airport Operations.
LEGAL CONSIDERATIONS: This item has been reviewed by the County Attorney's Office,
requires majority vote, and is legally sufficient for Board action. — CMG
GROWTH MANAGEMENT IMPACT: There is no Growth Management Impact associated
with the Executive Summary.
RECOMMENDATION: That the Board of County Commissioners, acting as the Airport
Authority, approve and authorize its Chairman to execute the attached Advertisement License
Agreement with Seminole Gaming Management, LCC for advertising at the Immokalee
Regional Airport.

THIS LICENSE AGREEMENT is between Advertiser and the Collier County Airport Authority.
1. Advertiser is hereby granted a license to advertise on the County's window of the Immokalee Regional Airport
terminal, 165 Airpark Boulevard, immokalee, Florida 34142.
2. The Advertiser may display the advertisement for a one (1) year period from the date that Advertiser's advertisement
is first displayed. If this License Agreement is not terminated by either party as set forth in Section 3, the Agreement
shall continue on a month to month basis. The Advertiser may occupy one (11 window in the terminal.
3. The Advertiser may display its advertisement on one window beginning on . 2012 and until at
least ten (10) days' advance written notice to terminate is given by one party to the other. Written notice from the
County can be mailed to Advertiser's "correct mailing address."
4. Advertiser agrees to pay (each twelve (12 ) months in advance) the Collier County Airport Authority S1.944.00
annually plus applicable sales tax for this license.
S. Advertiser will be responsible for the maintenance and change outs to its advertisements on the window.
6. The Authority shall have the sole and exclusive right to reject, for any valid reason, any advertisement submitted to be
displayed on the window. In that event, the Advertiser will be given an equitable pro -rata refund of license fees then
tendered to the Authority.
7. This agreement shall be the entire, full and complete agreement between the Advertiser and the Authority, and shall
supersede all prior written or verbal agreements. There are no valid or binding representations or promises between
the parties not embodied in this license agreement. No amendment, change or variance from this license agreement
shall be binding on either party unless executed in writing and signed by both parties.
B. Collier County Airport Authority is not responsible for damage to or condition of any of Advertiser's advertisements.
All advertisements are to be displayed at Advertiser's risks.
9. Collier County Airport Authority reserves the right to revoke this License at any time in its sole discretion by written
notice. Should the Authority exercise this right, Advertiser will immediately remove Its advertising from the premises,
and the Authority will issue a pro-rata refund to Licensee of its payment. Advertiser agrees that the issuance of a pro-
rata refund is Advertiser's sole remedy for such termination.
