EXECUTIVE SUMMARY
Recommendation to approve an amendment to the Home Care for the Elderly (HCE) agreement
between Collier County and the Area Agency on Aging for Southwest Florida, Inc. d/b/a Senior
Choices of Southwest Florida increasing funding by transferring funds between budget lines for
year two(2013-2014). (Net fiscal impact is $6,254)
OBJECTIVE: To provide uninterrupted support services to Collier County Services for Seniors (CCSS)
frail, elderly HCE clients.
CONSIDERATIONS: An amendment to the Home Care for the Elderly(HCE)agreement was received
from the Area Agency for Southwest Florida, Inc. d/b/a Senior Choices of Southwest Florida (Senior
Choices) increasing Case Management funding by $7,500 through a transfer of $1,246 from Special
Subsidy funding and $6,254 from Basic Subsidy funding with an effective date of November 1,2013. The
Basic Subsidy budget is retained and directly administered by Senior Choices for the benefit of qualified
Collier County clients; therefore the budget administered directly by Collier County will realize a net
increase of$6,254.00.This does not change the overall program funding.

*The HCE project includes $22,896 retained and directly administered by Senior Choices for basic
subsidies;the decrease of$6,254 to that budget line does not directly impact the County budget.
FISCAL IMPACT: A budget amendment is required to recognize increased grant revenue of$6,254.00
for the 2013-14 HCE Program in Human Services Grant Fund(707),Project 33272
GROWTH MANAGEMENT: There is no growth management impact from this recommendation.
LEGAL CONSIDERATIONS: This is a standard form amendment provided by Senior Choices of
Southwest Florida.This item is legally sufficient and requires a majority vote.-JAB
RECOMMENDATIONS: To approve an amendment and attestation statement between Collier County
and the Area Agency on Aging for Southwest Florida,Inc. d/b/a Senior Choices of Southwest Florida and
authorize the associated budget amendment.

This AMENDMENT_ entered into between the Area Agency on Aging for Southwest Florida. Inc. dba
Senior Choices of Southwest Florida, hereinafter referred to as the "Agency," and Collier County Board.
of County Commissioners, hereinafter referred to as the "Recipient,"amends contract HCE 203.12—_.
July 2012-June 2014.
The purpose of this amendment is transfer in Collier County $6,254 from Basics and $1,246 from Specials to
Case Management for year two, 2013-2014, or the rate schedule, subject to the availability of funds: and revise
ATTACHMENT VHI.
This amendment shall be effective November 1, 2013.
All provisions in the agreement and any attachments thereto in conflict with this amendment shall be and
are hereby changed to conform with this amendment.
All provisions not in conflict with this amendment are still in effect and are to be performed at the level
specified in the agreement.
This Amendment, two pages, is hereby made a part of this agreement.
